摘  要:目的评价在健康人群中,血清胱抑素C(CysC)作为肾功能指标是否优于血清肌酐。方法采用随机整群抽样的方法,招募北京15个社区自我评价健康人群,经体格检查、健康问卷调查、血常规、尿液分析和血液生化检测,共筛选出健康成年人324例,年龄20~86岁。采用免疫散射比浊法检测血清CysC,双血浆法检测肾小球滤过率(GFR)。结果以GFR 90 mL/(min·1.73 m2)为截点作ROC曲线,血清CysC的ROC曲线下面积大于血清肌酐,P<0.01;单独应用血清CysC和血清肌酐的R2分别为0.375和0.146,校正年龄后的R2分别为0.587和0.588。结论血清CysC评价健康成人肾功能优于血清肌酐,用血清CysC评价肾功能时纳入年龄因素,可能会改善其预测效能。Objective To study whether Cystatin C (CysC) are superior to serum creatinine in estimating kidney function in healthy adults. Methods Based on cluster sampling, we performed physical examinations, health question- naire surveys, blood routine tests, urinalysis, and biochemical detections, and then selected 324 healthy adults, aged 20 to 86, in Beijing. Serum CysC levels were measured by immune nephelometry, tow-sample method measurement of glomerular filtration rate (GFR) was performed. Results Using GFR 90 mL/( min ~ 1.73 m2 ) for the cut-off point to construct ROC curves, the AUC values for serum CysC level was higher than for serum ereatinine level, P 〈 O. 01. Multiple linear regres- sion analysis showed that the R2 ( adjusted for age) improved from O. 375 to 0. 587 for serum CysC level, and from 0. 146 to 0. 588 for serum creatinine level. Conclusions CysC in estimating kidney function is superior to serum creatinine in healthy adult. When evaluating renal function with serum CysC, age should be included to improve forecast of renal function.
