以腹痛为主诉无呼吸系统症状肺炎46例临床分析

摘　　要：目的提高对以腹痛为主诉无呼吸系统症状肺炎的认识。方法对南昌市第一医院呼吸内科2007年5月至2011年8月住院确诊为腹痛型肺炎的46例患者的临床情况进行综合分析。结果患者临床表现为腹痛,以老年人及下肺炎症多见,无呼吸系统症状,胸部早期无异常体征。结论肺炎,尤其是下肺炎症,可以表现为腹痛,而无呼吸系统症状及体征,因此对于原因不明的腹痛患者,尤其是老年人,虽然无呼吸系统症状,亦应认真进行胸部体检,同时常规胸片,必要时胸部CT平扫,以免贻误诊治。Objective To improve the understanding of pneumonia with the main complaint of abdominal pain and non- respiratory symptoms. Methods Inpatient cases diagnosed as pneumonia with abdominal pain were analyzed comprehensively from May 2007 and August 2011. Results The clinical manifestations were abdominal pain which were more common in elderly people and pneumonia in lower lung, non-respiratory symptoms, no abnormal chest signs early. Conclusion Pneumonia, especially pneumonia in lower lung, can be expressed as abdominal pain, and without respiratory symptoms and signs. For patients with unexplained abdominal pain, especially the elderly people, although no respiratory symptoms, chest examination should be careful while conventional chest X-ray, chest CT scan if necessary, to avoid delaying treatment.
