胰岛素抵抗与血清GGT的相关性研究

摘  要:目的探讨2型糖尿病患者胰岛素抵抗(IR)与血清γ-谷氨酰转移酶(GGT)的关系。方法选择2型糖尿病患者109名,并根据正常对照组所得胰岛素抵抗指数(HOMA.IR)的中位数为标准,将其分为胰岛素抵抗组(IR)组55例,和非胰岛素抵抗组(DM)组54例。同时选取30名健康体检者组成正常对照组(NC)组。各受试对象分别测定空腹血糖(FBG),空腹胰岛素(FINS),血清GGT及CRP等。结果 IR组GGT明显高于DM组和NC组(P<0.05),IR组和DM组的CRP均明显高于NC组(P<0.05)。相关分析显示,胰岛素抵抗指数与血清GGT呈正相关(P<0.05)且血清GGT与CRP,TG等均呈正相关(P<0.01)。结论血清GGT水平可能与胰岛素抵抗有密切关系,其检测有助于临床对胰岛素抵抗的诊断和治疗。Objective To investigate the relationship between insulin resistance (IR) and serum gamma- glutamyl transferase (GGT) level in patients with type 2 diabetes. Methods A total of 109 patients with type 2 diabetes were enrolled in the study; meanwhile, 30 normal subjects were selected as a normal control (NC) group. The patients were divided into IR group (n = 55) and non-IR group (n = 54) according to the median value of IR index (homeostasis model assessment of insulin resistance, HOMA-IR) in NC group. The fasting blood glucose, fasting insulin, and serum GGT and C-reactive protein (CRP) levels were measured. Results The serum GGT level in IR group was significantly higher than those in non-IR group and NC group (P〈0.05). The serum CRP levels in IR group and non-IR group were significantly higher than that in NC group (P〈0.05). The correlation analysis showed that there was positive correlation between HOMA-IR and serum GGT level (P〈O.05), and the serum GGT level was positively correlated with serum CRP and triglyceride levels (P 〈0.01 ). Conclusion Serum GGT level may be closely related to IR. The determination of serum GGT level is helpful to the diagnosis and treatment of IR.
