单孔法与多孔法腹腔镜肝囊肿去顶减压术的回顾性对照研究

摘　　要：目的:探讨经脐单孔腹腔镜肝囊肿去顶减压术在单纯性肝囊肿治疗中的可行性和安全性。方法:回顾总结2009年5月—2011年4月12例接受经脐单孔腹腔镜肝囊肿去顶减压术患者的临床资料。选取同时期于我院接受传统多孔法腹腔镜肝囊肿去顶减压术的12例年龄相近,囊肿性质、位置相似的病例进行对照研究。比较两组的手术时间、术中出血量、胃肠功能恢复时间、术后引流量、术后引流时间、术后住院时间、术后复发情况等指标。结果:单孔组与多孔组在手术时间(55.5±31.3)min vs(58.3±26.7)min、术中出血量(13.8±8.0)mL vs(13.3±6.2)mL、胃肠功能恢复时间(2.5±0.8)d vs(2.4±0.9)d、术后引流量(448.6±136.3)mL vs(450.0±155.3)mL、术后引流时间(3.9±1.3)d vs(4.6±1.8)d、术后住院时间(4.5±1.7)d vs(5.1±2.4)d等方面无显著差异(P>0.05)。术后随访时间为1～24个月,单孔组除1例患者原有肝内小囊肿增大外,未见复发病例。结论:与传统多孔法腹腔镜肝囊肿去顶减压术比较,单孔腹腔镜手术操作虽略复杂,但总体治疗过程无明显差异。在兼具创伤小,康复快,住院时间短等优点的同时,单孔腹腔镜手术还可以达到"无瘢痕"的效果,可以安全有效的开展。Objective： Aim of this report was to documents gle-incision laparoscopic livercyst fenestration in the treatment of spective case-control study. Methods： From May 2009 to Ap the feasibility and safety of sinsimple hepatic cysts by a retroril 2011,12 cases accepted single-incision laparoscopic liver cyst fenestration. 12 simple liver cyst cases with similar age, cyst nature and position to the single-incision group were selected during the same period in our hospital to receive the traditional multi-incision laparoscopic liver cyst fenestration. Results： There was no significant difference between single-incision and multi-incision group in the operation time （,55.5 ± 31.3）min vs （58.3 ± 26.7）min, blood loss （13.8 ± 8.0）mL vs（13.3 ± 6.2）mL, recovery time of gastrointestinal function （2.5 ＋ 0.8）d vs （2.4 ＋ 0.9）d, volume of postoperative drainage （448.6 ± 136.3） mL vs 450.0 ± 155.3）mL, postoperative drainage time （3.9 ± 1.3）d vs （4.6 ± 1.8）d, postoperative hospitalization time（4.5 ± 1.7）d vs （5.1 ± 2.4）d （P〉0.05）. Conclusions： Compared with traditional multi-incision laparoscopic liver cyst fenestration, though the single-incision laparoscopic surgical procedure was slightly complicated, no significant difference was found between them in the overall treatment process.
