运用中医外治三联模式治疗0级糖尿病足40例临床观察

摘　　要：目的:观察中医外治三联模式(穴位贴敷、穴位注射及中药足浴)治疗0级糖尿病足之临床疗效及其安全性。方法:按Wagner分级法将符合0级糖尿病足的住院患者80例,随机分成治疗组及对照组,每组40例,对照组给予基础治疗,治疗组则在基础治疗上联合运用穴位贴敷、穴位注射及中药足浴。两组分别在治疗前及治疗2周后进行评价,并对其结果进行统计分析。结果:两组患者疗效比较,治疗组40例中,基本痊愈6例,显效16例,有效15例,无效3例,总有效率为92.5%,对照组40例中则分别为2例、8例、17例及13例,总有效率为67.5%,两组比较差异有统计学意义(P<0.05),治疗组优于对照组。治疗组出现1例足背皮肤轻微烫伤,嘱咐患者适当调整足浴水温可缓解,并继续坚持治疗;其余患者未见不良反应。结论:中医外治三联模式治疗0级糖尿病足,安全有效,操作简单,值得推广。Objective： To evaluate the curative effect and safety of point-application, point injection, foot massage with Chinese medicine for diabetic foot with grade 0. Methods： Totally 80 patients were randomized into treatment group and control group with forty in each. The treatment group accepted point-application, point injection, foot massage with Chinese medicine, while the control group accepted routine treatment of diabetic foot. Both groups were scored before treatment and at the 14th after the treatment. Then the score results were statistically analyzed. Results： The clinically-controlled, excellent, effective and ineffective cases were 6, 16, 15 and 3 in the treatment group, while 2, 8, 17 and 13 respectively in the control group. The total effective rates of the treatment group and control group were 92.5% and 67.5% respectively, the difference between was statistically significant（P〈0.05）. During the treatment, we observed 1 case scalded slightly on the back of the foot skin in treatment group; after advised to adjust the temperature of water solution, he persisted in the treatment. Conclusion： Point-application, point injection, foot massage with Chinese medicine was safe, effective for diabetic foot with grade 0, meanwhile, it was simple operation and worthy of promotion.
