经下腹横切口腹膜前间隙无张力修补术治疗成人腹股沟疝临床分析(附46例报告)

摘　　要：目的探讨下腹横切口腹膜前间隙无张力修补术治疗腹股沟疝的可行性及临床疗效。方法收集分析川北医学院附属医院2010年6月至2012年1月采用下腹横切口腹膜前间隙无张力修补术式的成人腹股沟疝患者46例临床资料,观察疗效及术后并发症等。结果术中腹膜小破损5例,腹壁下血管损伤出血1例,术后尿潴留4例,阴囊积液3例,阴囊或大阴唇水肿8例,肺部感染1例;随访6～18个月,无复发。结论下腹横切口腹膜前间隙腹股沟疝无张力修补术,经后入路Bogros间隙置入补片,达到耻骨肌孔的全覆盖,符合生理解剖,手术易操作,复发率低,并发症少,美容效果好,尤其适合双侧疝及复发疝患者,是一种安全可靠的无张力疝修补术。Objective To evaluate the feasibility and effectiveness of preperitoneal space tension- free hernioplasty through inferior abdominal transverse incision for inguinal hernia. Methods A total of 46 patients with adult inguinal hernias from June 2010 to January 2012 in Affiliated Hospital of North Siehuan Medical College were enrolled. All the patients were treated with the open preperitoneal space tension-free hernioplasty through inferior abdominal transverse incision. The curative effect and postoperative complica- tions were observed. Results The rupture of abdominal membrane was found in 5 patients, angiorrhexis of abdominal wall in 1 patient, retention of urine in 4 patients, scrotal hydrocele in 3 patients, edema of scrotum （greater lip of vulva）in 8 patients and pulmonary infection in 1 patient. No recurrence was occurred in the follow-ups for 6 to 18 months. Conclusions The preperitoneal spaee tension-free hernioplasty through inferi- or abdominal transverse incision for inguinal hernia repair based on the theories of anatomy and physiology, was proved to be effective, safe, and convenient, especially for those with bilateral and recurrent hernias.
