曲美他嗪联合生脉注射液、硝普钠治疗难治性心力衰竭临床观察

摘　　要：目的:评价曲美他嗪联合生脉注射液、硝普钠治疗难治性心力衰竭(RHF)临床疗效。方法:96例RHF患者随机分为对照组和治疗组。对照组48例采用常规治疗;治疗组48例在对照组治疗的基础上加用曲美他嗪、生脉注射液、硝普钠治疗。观察治疗前后心功能(NYHA)分级、6min步行距离、心脏超声指标和血浆N端脑钠肽前体(NT-proBNP)水平。结果:治疗组总有效率、显效率分别为91.7%和52.1%,显著高于对照组的77.0%和25.0%(P<0.05,P<0.01);治疗组治疗后LVEF、E/A较对照组明显改善,TN-proBNP水平明显降低,6MWT距离明显延长(P<0.05,P<0.01),治疗组心功能及临床症状的改善均较对照组显著(P<0.05),未见严重的不良反应。结论:在常规治疗基础上加用曲美他嗪、生脉注射液、硝普钠能改善RHF的心功能、提高生活质量。Objective： To evaluate the clinical efficacy of combination of trimetazidine with shengmai injection,nitroprusside on refractory heart failure（RHF）patients. Methods： 96 patients with RHF were randomly divided into the control group（n.=48） and the treatment group （n=48） in which the control group received only routine treatment of refractory heart failure and the treatment group received routine treatment plus trimetazidine with Shengmai injection, nitroprusside. Changes of cardiac functional grading, 6-min walking distance,echoeardiographic indices, plasma N terminal pro-bmin natriuretic peptide （NT-proBNP） level were respectively observed before and after treatment. Results： The total effective rate and obvious effective rate respectively were 91.7% and 52.1% in the treatment group, significantly higher than those（77.0 % and 25.0% ） of the control group（P〈 0.05, P〈0.01 ）. The improvemen[ of clinical symptoms and heart function in the treatment group were significantly better than that in the control group（P〈0.O5）.LVEF,E/A apparently improved after treatment in the treatment group comparing with the control group, and dec,eased NTproBNP level, the distance in 6MWT extended（P〈0. 05, P〈0.01 ）. No serious side effect was found. Conclusion： Based on the conventional treatment,the adjuvant therapy of trimetazidine with Shengmai injection, nitroprusside could improve the cardiac function, and the quality of life in patients with RHF.
