摘 要:目的探讨甲状腺滤泡型乳头状癌(FVPTC)的超声表现。方法回顾性分析经手术和病理证实的36例FVPTC患者的超声表现,其超声特征与43个甲状腺良性结节进行对照分析。结果 36例FVPTC的超声表现可分为3种类型:(1)Ⅰ型6例(16.7%),具备甲状腺乳头状癌的典型超声表现,结节形态不规则,边界不清,内部可呈极低回声,可见微小钙化;(2)Ⅱ型15例(41.7%),表现为境界较清晰的等回声或低回声结节,其形态不规则,边缘可见成角或分叶,微小钙化较少见;(3)Ⅲ型15例(41.7%),声像表现形似腺瘤,结节边界清晰且边缘光整,内部为均匀中等回声。与43个甲状腺良性结节对比,FVPTC超声表现为形态不规则(52.8%)、实性结节(97.2%)、微小钙化(19.4%)以及可探及肿大淋巴结(25%),上述超声特征的显示率要高于甲状腺良性结节,差异有统计学意义(P<0.05)。结论不同病理亚型的FVPTC超声表现有很大差异,需要充分了解其声像图特征,方能提高对FVPTC的诊断符合率。Objective To investigate the ultrasonic appearances of follicular variant of papillary thyroid carcinoma (FVPTC). Methods Ultrasonic appearances of 36 patients with FVPTC confirmed by surgery and pathology were analyzed retrospectively and compared with 43 benign thyroid nodules. Results Ultrasonic appearances of 36 cases of FVPTC could be classified into three types: (1) type Ⅰ 6 cases (16.7 %), with typical sonographic features of papillary thyroid carcinoma: nodules with irregular shape, unclear boundary, and very low echo inside, microcalcification could be seen. (2) type Ⅱ15 cases (4:1.7 %), ultrasonic performance: clearer iso-echoic or hypo-echoic nodules with irregu- lar shape, angled and lobular edge, but rarely microcalcification. (3) type Ⅲ 15 cases (41.7%), ultrasonic performance: they look like thyroid adenoma, with clear boundary and regular edge, uniform medium echo inside. Contrasting with 43 benign thyroid nodules, the ultrasonic appearances of FVPTC showed irregular shapes (52.8%), solid nod ules (97.2%), microcalcifications (19.4%), enlarged lymph nodes (25%) which were higher than benign thyroid nodules, and the differences were statistically significant(P〈0.05). Conclusions Different pathological subtypes of FVPTC have different sonographic features. It is imperative to know more sonographic features about them to improve the diagnosis accuracy rate of FVPTC.
