超声弹性成像在干燥综合征诊断中的应用价值

摘　　要：目的探讨超声弹性成像在干燥综合征(sjogren's syndrome,SS)诊断中的应用价值。方法入选72例检查对象,其中病例组37例,对照组35例,分别进行双侧腮腺及颌下腺超声弹性成像检查,测量腺体与附近肌肉的应变率比值(strain ratio,SR),以SPSS 19.0进行统计学分析。结果腮腺、颌下腺的SR与SS病理分期的相关系数,分别为0.81、0.84(P<0.05)。腮腺、颌下腺诊断早期SS的SR临界值,分别为1.20、1.23,诊断中晚期SS的SR临界值,分别为1.90、1.87。结论腮腺、颌下腺超声弹性成像参数与SS病情进展呈正相关关系,超声弹性成像有助于判断涎腺受累程度,对SS的诊断具有较高的应用价值。Objective This research is to explore the application value of ultrasound elastography in the diagnosis of SS （Sjogren＇s Syndrome）. Methods 72 subjects（37 patients, 35 healthy persons）were enrolled in this research, their parotid gland and submaxillary gland were examined by ultrasound elastography. The SR （Strain Ratio）of the gland and the nearby muscle was measure. The SPSS 19.0 software were used to analyze the data. Results There was a high consistency between parotid gland and submaxillary gland＇s SR and the pathological staging of SS, the correlation coefficient were 0.81,0.84 respectively（P〈0.05）. To diagnose the early stage of SS,the SR of parotid gland and sub- maxillary gland were 1.20,1.23 respectively. To diagnose the advanced stage of SS, the SR of parotid gland and sub maxillary gland were 1.90,1.87 respectively. Conclusions There was a positive correlation between salivary glands＂ SR and the pathological staging of SS. Uhrasound elastography is helpful to evaluate status and is valuable in diagnosis of SS.
