超声射频信号技术分析正常妊娠妇女颈动脉血管功能的研究

摘　　要：目的采用超声射频信号血管分析技术对正常妊娠妇女颈动脉的弹性进行综合分析评估并与育龄妇女进行对比,观察正常妊娠妇女颈动脉血管功能变化,为进一步研究妊娠合并症血管功能提供依据。方法正常不同孕周妊娠妇女77例,年龄匹配正常育龄妇女30例,应用百胜公司彩色多普勒彩色诊断仪Mylab70XV,探头型号LA523,分别启用血管内-中膜分析技术(QIMT)和血管硬度定量分析技术(QAS)分析颈动脉结构及僵硬度。结果 (1)正常妊娠妇女颈总动脉最大及最小血流速度、阻力指数(RI)、搏动指数(PI)、扩张系数(DC)、膨胀系数(CC)、动脉压增强指数(AIx)均显著低于育龄组(P<0.01);硬度指数(α、β)、脉搏波传播速度(PWV)、收缩压(SP)、平均动脉压(MAP)、等容收缩期到射血期转折点压力(PT_1)及动脉增强压(AP)均显著高于育龄组(P<0.01)。(2)随孕周增加,正常妊娠妇女颈总动脉最大及最小血流速度、SP、MAP、PT_1、PWV逐渐升高(P<0.05)和RI逐渐降低(P<0.05)。结论与育龄妇女相比,正常妊娠妇女颈动脉弹性减低而硬度显著增高。随孕周增加,正常妊娠妇女血管硬度显著升高,内中膜厚度(IMT)无显著差异。Objective To evaluate the carotid arterial stiffness and elasticity in normal pregnancy using ultrasound radio-frequency technique. Methods Seventy-seven normal pregnant women were included and thirty agematched non-pregnant healthy women severed as controls. Carotid arterial structure, stiffness and cardiac function were evaluated using Quality Intima-Media Thickness （QIMT） and Quantitative Artery Stiffness （QAS） techniques. Results Carotid blood pressure, maximal and minimal carotid blood flow velocities and stiffness were significantly higher in pregnancy group than those in the control group（P〈0.01）. Common carotid MAX and MIN flow velocities, blood pressures and arterial stiffiness parameters,including pressure at T1, pulse wave velocity increased with gestational age （P〈0.05）. No significant difference was found in IMT between normal pregnant women and controls. Conclusions The common carotid elasticity during pregnancy is decreased and the stiffness is increased apparently comparing with the reproductive age women,while IMT has no difference. Common carotid MAX and MIN blood flow velocities and stiffness increase with gestational age.
