摘  要:目的探讨超声血流动力学参数在预测低危妊娠滋养细胞肿瘤患者首次化疗时对氨甲喋呤(MTX)敏感性的价值。方法低危妊娠滋养细胞肿瘤患者首次化疗前检测其血清人绒毛膜促性腺激素(β-HCG)水平,并行彩色多普勒超声检查,测量其子宫动脉PI值和RI值。所有患者均采用氨甲喋呤(MTX)单药首次化疗。化疗开始后每周检测其血清β-HCG水平。根据血清β-HCG下降程度分为敏感组和不敏感组。比较两组的子宫动脉血流动力学参数是否有统计学差异。结果敏感组和不敏感组的子宫动脉血流动力学参数有统计学差异(P<0.05)。结论对于首次化疗的低危妊娠滋养细胞肿瘤患者,超声检测子宫动脉的血流动力学参数可以为其对MTX化疗的敏感性提供一定的预测参考。Objective To evaluate the value of ultrasonic hemodynamics parameters in predicting sensitivity of primary chemotherapy with single methotrexate on low-risk gestational trophoblastic neoplasia. Methods Detect hu- man chorionic gonadotropin(β-HCG)in serum of patients with low-risk gestational trophoblastic neoplasia before chem- otherapy, and give them Color Doppler Ultrasound examination to get PI and RI values of uterine artery. All patients were treated with single methotrexate(MTX) for the first time chemotherapy. Test β-HCG level in serum weekly after the start of chemotherapy. All patients are divided into MTX-sensitive group and MTX-insensitive group according to the drop degree of β-HCG level. The ultrasonic hemodynamics parameters in two groups are compared. Results The ultrasonic hemodynamics parameters in two groups are statistically significant(P〈0.05). Conclusions For patients on low-risk gestational trophoblastic neoplasia treated with single methotrexate(MTX)chemotherapy for the first time, ultrasonic hemodynamics parameters of uterine artery can provide a reference in predicting sensitivity.
