胰腺实性假乳头状瘤声像图特征及超声诊断分型初探

摘　　要：目的探讨胰腺实性假乳头状瘤(SPTP)的声像图特征及超声诊断分型。方法分析手术、病理证实11例SPTP的超声表现和相关资料,根据SPTP囊、实成分比例拟定分型标准。结果 11例SPTP均为单发圆形、椭圆形实性、混合性略低回声团;9例(9/11)边界较清楚,8例(8/11)包膜完整,1例(10/11)瘤后回声无衰减,9例(9/11)相邻组织器官受压、移位;除1例胰头SPTP远端胰管略宽外(0.3 cm),余患胰、胆管均无扩张;8例(8/11)瘤内及周边可见少许血流信号。SPTP超声分型:Ⅰ型(实质型)2例;Ⅱ型(混合型)9[其中Ⅱa型(以实为主)5例,Ⅱb型(囊、实参半)3例,Ⅱc型(以囊为主)1例];Ⅲ型(类囊型),本组无此病例。11SPTP的超声表现与术中所见、病理结果基本一致。结论 SPTP的超声表现有一定的特征性;统一的分型标准便于规范SPTP特征描述,对其发病规律观察和学术交流与合等有重要意义。Objective To study the ultrasonographic characteristics and type of ultrasound diagnosis for Solid Pseudopapillary Tumor of Pancreas（SPTP）. Methods The ultrasonic manifestations and related information of 11 cases of SPTP were analysis by operation and pathology. The type standard was prepared according to SPTP capsule, solid composition. Results 11 cases SPTP were solitary circular, elliptical or mixed slightly low echo mass, 9 cases had clear boundary（9/ll）. 8 cases had completive encapsulatad（8/ll）, 10 cases showed no echo attenuation after tumor（10/ll）, 9 cases showed adjacent tissue or organs compression, displacement（9/11）, Except for 1 case in pan- creas head showed distal pancreatic duct widen （0.3 cm）, others showed no pancrestic duct and bile duct dilatation. 8 cases found little blood flow in and around the tumor（8/11）. The ultrasonic type： type I （solid type） 2 cases, type Ⅱ （mixed type） 9 cases Ⅲ type A （mainly solid） 5 cases,Ⅱ type B （cystic and solid mixed） 3 cases, Ⅱ type C （mainly cystic） 1 case]. Type Ⅲ（cystoid type） no such case in this group. In 11 patients, SPTP ultrasound and intraoperative findings are consistent with pathological examination results. Conclusions SPTP ultrasound has certain characteris- tics. Unified classification criteria has important significance to standard SPTP feature,the pathogenesis and academic cooperation.
