宫腔镜联合药物治疗宫角妊娠18例分析

摘　　要：目的探讨宫腔镜联合米非司酮、米索前列醇片等药物治疗早期未破裂型宫角妊娠的疗效。方法对2008年1月-2011年6月确诊为早期未破裂型宫角妊娠的患者,应用米非司酮、米索前列醇片配合宫腔镜技术进行手术处理。宫腔镜术前先口服米非司酮片150 mg,48 h后阴道置米索前列醇片600μg,诱发子宫收缩、排出妊娠产物后根据复查超声结果选择性进行宫腔镜手术治疗,对患者临床资料进行回顾性分析。结果 18例早期未破裂型宫角妊娠患者均得以治愈,无一例出现子宫穿孔、破裂、腹腔内出血等并发症。18例患者随访6～36个月,血β人绒毛膜促性腺激素(HCG)逐渐转阴,平均术后30～40 d月经正常复潮,12例有生育要求者7例已妊娠分娩。结论宫腔镜联合米非司酮、米索前列醇片等药物治疗早期未破裂型宫角妊娠是一种安全、有效、微创且经济的方法,值得临床推广。Objective To investigate the curative effect of medical abortion such as mifepristone and misoprostol combined with hysteroscope among the early comual pregnancy which is not cracked. Methods From January 2008 to June 2011, the med-ical abortion such as mifepristone and misoprostol combined with hysteroscope was given to the patients whose illnesses were diag- nosed as early corneal pregnancies that was not cracked. Misoprostol of 600 ug can be administered into the patient＇ s vaginal and fallowed later by taking 150 mg mifepristone tablets orally to induce uterine contractions to expel the products of conception. To retrospectively analyse the matched patients who have been treated by hysteroscope according to the result of ultrasound. Results The 18 patients in the research has all been cured,and there are no complications such as uterine perforation,hysteror-rhexis, intra-abdominal hemorrhage and so on. Follow-up visiting all these patients in 6-36 months ,we found blood βhuman gon-adotropins would turn down to the normal level gradually, and menstruation would become normal in 30-40 days. 12 patients in this group who wanted to have a baby has deliveried. Conclusion The medical abortion combined with hysteroscope to cure the patients whose illnesses were diagnosed as early corneal pregnancies that was not cracked was a safe, effective, and minimal inva-sion and economic method, which was worthy to be extended in the clinical treatment.
