血清可溶性髓样细胞触发受体-1水平与社区获得性肺炎不同中医证型的相关性研究

摘　　要：目的研究血清可溶性髓样细胞触发受体-1(sTREM-1)水平及其他炎症因子与社区获得性肺炎中医证型的关系,探索中医辨证分型的客观指标。方法选择社区获得性肺炎患者91例为感染组,对感染组患者进行中医辨证分型;以同期健康体检者30例为对照组。所有受检者检测血常规,测定C反应蛋白(CRP)和血沉(ESR),留取血清标本,酶联免疫吸附法(ELISA)检测sTREM-1水平并进行比较。结果社区获得性肺炎患者的各炎性指标均明显高于对照组(P均<0.05);社区获得性肺炎患者血清炎症指标在各证型之间有一定的变化规律,WBC、N、ESR、CRP、sTREM-1水平的值由低到高顺序依次为肝火犯肺证、痰热郁肺证、痰湿蕴肺证,ESR在各组之间比较无显著性差异,痰热郁肺证和肝火犯肺证患者CRP、sTREM-1比较无显著性差异。结论炎性指标WBC、N、ESR、CRP、sTREM-1有作为判断社区获得性肺炎中医证型参考指标的可能。Objective It is to study the relationship between syndrome pattern of TCM and sTREM - 1 and other inflamma- tory markers in community acquired pneumonia （CAP） , to search the new objective marks so that to lay the foundation for the promotion of Chinese medicine. Methods 91 patients suffered from CAP were divided into infection group, identify the TCM syndrome types of infection group. 30 health volunteers were chosen as control group. Clinical date, including routine blood test, CRP and ESR were determined in all subjects. The level of sTREM - 1 was measured by enzyme linked immunoabsorbant assay （ELISA） and the differences were compared between groups. Results The level of various inflammatory markers were significantly higher in the patients with CAP than those of controls（ P 〈 0.05 ）. There were some change rules in the number of the inflammatory markers among syndrome pattern of TCM in CAP, the number of WBC, N, ESR, CRP and sTREM - 1 （ from low to high ） ：Ganhuofanfei group 〈 Tanreyufei group 〈 Tanshiyunfei group. It showed no significant difference in the num- ber of ESR between all kinds of syndrome pattern（ P 〉 0.05 ） , it showed no significant difference in the number of CRP and sTREM - 1 between Tanreyufei and Ganhuofanfei pattern （ P 〉 0.05 ）. Conclusion Inflammatory indexes such as WBC, N, ESR, CRP,sTREM -1 may become judgment TCM reference index in CAP.
