射频消融迷宫术治疗心房纤颤66例

摘　　要：目的　 Cox从 1991年采用手术切割加冷冻方法治疗心房纤颤 ( AF) [1 ] ,但手术操作复杂 ,有潜在术后出血的危险性 ,使这一方法的使用受到一定限制 ,为此 ,我们采取措施以克服上述不足 .方法　 1995 - 0 4/ 1999- 0 4我们在为 6 6例风湿性二尖瓣病患者施行瓣膜置换时 ,采用心内直视射频消融迷宫术对其合并的 AF予以治疗 . 6 6例中男 30例 ,女 36例 ,平均年龄 43.5岁 .AF病史最短 1a,最长 30 a,平均 3.6 a.射频消融迷宫术原则上采用小板井嘉夫迷宫手术路线 [2 ] .结果　以出院时为标准 ,有 5 4例恢复了窦性心律 ,占 82 % ,9例仍为 AF,2例为心房朴动 . 1例为结性心律 .结论　射频消融迷宫术治疗AF方法简单 ,效果满意 。AIM Modify Cox maze procedure and make it can be adopted in common use without complex technique and possible big bleeding after operation. METHODS We treated 66 cases of atrial fibrillation (AF) with rheumatic heart disease using radiofrequency ablation maze procedure as valve replacement were performed. 30 cases of 66 cases were male, 36 female, ages of the patients were from 30 to 58 years old (average 43.5 years). The duration of AF was from 1 year to 30 years (average 3.6 years). We adopted Shio Kosakai'smaze procedure route in radiofrequency ablation in principle. RESULTS 54 cases of 66 cases resumed sinus rhythm (82%), 9 patients remained AF, 2 patients turned to atrial flutter, 1 patient was junctional rhythm when the patients dischaged. CONCLUSION Radiofrequency ablation maze procedure is a simple, safe and effective method to treat atrial fibrillation.
