乌鸡白凤丸联合二甲双胍治疗多囊卵巢综合征的实验研究

摘　　要：目的探讨中药乌鸡白风丸（BFP）联合二甲双胍（MET）对多囊卵巢综合征（polycystic ovarian syndrome，PCOS）大鼠的治疗作用。方法应用DHEA建立SD大鼠PCOS模型，随机分为模型空白组（B组）、造模后乌鸡白风丸治疗组（C组）、造模后二甲双胍治疗组（D组）、造模后乌鸡白风丸联合二甲双胍治疗组（E组），另设溶媒对照组（A组），每组10只。C、D、E组给了相应的药物灌胃。每日阴道涂片观察各组大鼠排卵情况，连续灌胃3周后，取出卵巢、子宫、肾上腺称重。一侧卵巢、肾上腺置于10％的福尔马林溶液中固定，另一侧置于液氮中速冻后于70℃低温冰箱保存待测。观察各器官大体形态学及光镜（HE染色）特征，检测空腹血糖（FBG），测定血清睾酮（T）、黄体生成激素（LH）、空腹胰岛素（FIN）、胰岛素样生长因子-1（IGF-1）的水平，计算稳态模型胰岛素抵抗指数（HOMA—IR）。结果联合用药组与模型空白组比较T、FIN、FBG、HO—MA—IR均降低，差异有显著性；联合用药组卵巢、肾上腺湿重较模型空白组减轻明显，子宫湿重与模型空白组比较承量增加，组织病理切片显示各组织形态学良性恢复改变。各治疗组均有大鼠恢复排卵，其中联合用药组恢复排卵早，排卵周期多。结论中药乌鸡白风丸（BFP）联合二甲双胍（MET）可有效降低外周血T水平，降低HOMA—IR，促使PCOS卵巢恢复排卵，治疗多囊卵巢综合征效果显著。Objective To investigate the therapeutical effect and the mechanism of BFP and metformin alone or in combination in the treatment of PCOS. Methods Female SD rats with PCOS model were randomly divided into five groups of 10 each, control group（ A ）, model group （B）, BFP group （C）, Metformin group （D）, associated medication group （E）, then intragastrie administration with medicine was given to each group respectively. The information of ovu- lation was recorded each day. Up to 3 weeks,the ovary,uterus and adrenal were weighed,gross anatomy and light mi- croscopic construction were performed. Fasting blood glucose （ FBG）, testosterone （ T）, luteinizing hormone （ LH）, fast- ing insulin （FIN） and serum insulin growth factor - 1 （ IGF - 1 ） levels were measured. Homeostasis model insulin re- sistance index（ HOMA - IR） was calculated. Results The levels of T, FIN, FBG, IGF - 1, HOMA - IR in the com- bined medication group were lower than that of the model blank group. The weight of ovary and adrenal in the associat- ed medication group were much lighter than that of the model blank group, but the weight of uterus was increased. The tissue recovered histomorpbologically after treating,whlch appeared the most in the associated medication group of all groups,such as the earliest ovulating,the most ovulatory cycles and so on. Conclusion The effect of BFP combined with metformin in the treatment of PCOS is significant. Combined with traditional Chinese medicine and western medi- cine may ＇affect the expressions of FIN, IGF - 1, then induce a remarkable effect to adjust genitical gland function. It shows the whole therapeutie action of Chinese medicine,and also provides a new feasible approach in the treatment of PCOS.
