芪苈强心胶囊治疗老年慢性心力衰竭的效果和安全性研究

摘　　要：目的观察芪苈强心胶囊对老年慢性心力衰竭患者的临床疗效。方法选取2010年2月-2011年12月在我科住院的老年慢性心力衰竭患者60例，随机分为常规治疗组（对照组）和芪苈强心胶囊治疗组（观察组），每组30例。对照组按心衰指南给予常规治疗，心力衰竭组在常规治疗的基础上加用芪苈强心胶囊，每日3次，每次4粒。共观察24周，观察治疗前后2组患者的心功能分级、左室射血分数（LVEF）值、左室舒张末期内径（LVDd）、脑钠肽以及肝肾功能、血尿酸、电解质等指标。结果①两组临床疗效比较：对照组显效10例（33．3％），有效12例（40．0％），总有效数22例（73．7％）；观察组显效12例（40．4％），有效16例（53．3％），总有效数28例（93．3％）。观察组总有效率明显高于对照组（P〈0．05）。②与治疗前比较，两组患者治疗24周后LVEF均有不同程度提高（P〈0．05，P〈0．01），LVDd和脑钠肽有所下降（P〈0．05，P〈0．01）；与对照组比较，观察组各项指标均改善更为明显（P〈0．05）。③与治疗前比较，观察组BUN、Cr、UA均明显下降（P〈0．05），而对照组BUN、Cr无明显变化，且血尿酸水平较治疗前增高（P〈0．05）；两组比较有显著性差异（P〈0．05）。结论芪苈强心胶囊能促进改善老年慢性心力衰竭患者的心、肾功能，无不良反应发生。Objective To observe the clinical efficacy of Qiliqiangxin Capsule on geriatrics who have chronic cardiac failure. Methods 60 patients from Feb. 2010 to Dec. 2011 were divided randomly into the test group（ Qil- iqiangxin Capsule）and control group（ regular treatment）, and 30 patients in each group. Regular treatment according to guideline of cardiac failure was given in both groups ,while Qiliqiangxin Capsule（4 grain each for 3 times per day） was added in the test group. The period last 24 weeks and record clinical indicator before and after the therapy, inclu- ding cardiac functional grading, left ventricle ejection fraction （LVEF） ,left ventricle diastolic diameter（LVDd） , brain natriuretic peptide,liver/renal function, blood uric acid, electrolyte and so on. Results 1 ） Clinical efficacy of two groups was compared：in test group, significant efficiency was with 12 cases （40.4%）, efficiency was with 16 cases （53.3 % ）, and total efficiency was with 28 cases （93.7%） ;in control group, significant efficiency was with 10 cases （33.3 % ）, efficiency was with 12 cases （40.0%）, total efficiency was with 28 cases （73.3%）. Test group had higher efficiency （ P 〈 0.05 ）. 2 ） After 24 weeks therapy in both group, LVEF was raised （ P 〈 0.05, P 〈 0.01 ）, and brain natriuretic peptide, LVDd were descended （ P 〈 0.05, P 〈 0. 01 ）, the amelioration of test group was more significant （ P 〈0.05）. 3）We compared the data before and after therapy, BUN, Cr, UA were distinctly descended in test group; BUN and Cr were with no evident change combined with increase of blood uric acid level （ P 〈 0.05 ）. Significant differences were between two groups （P 〈 0.05 ）. Conclusion Qiliqiangxin Capsule can improve the cardiac/kidney function and without any adverse reaction.
