腹腔镜改良全子宫切除术的临床应用价值

摘　　要：目的:探讨全子宫切除术的最佳术式。方法:回顾分析173例行全子宫切除术患者的临床资料,其中行传统经腹全子宫切除术(TAH组)68例,腹腔镜下全子宫切除术(LTH组)55例,腹腔镜改良全子宫切除术(LMTH组)50例,比较三组手术效果。结果:TAH组、LMTH组手术时间均明显少于LTH组(P<0.05);LMTH组的术中出血量、术后病率明显少于其他两组(P<0.05);TAH组术后疼痛时间、术后胃肠蠕动恢复时间、抗生素的使用时间、卧床时间、住院时间均明显长于LTH组、LMTH组(P<0.05)。LMTH组阴道残端肉芽发生率低于其他两组(P<0.05)。结论:LMTH具有创伤小、病人痛苦轻,术后恢复快的优点。LMTH安全性好,易于掌握,值得临床推广应用,有广阔的应用前景。Objective： To explore the best technique of total hysterectomy surgical procedures.Methods： The surgical effect of 173 cases of patients in three groups with total hysterectomy,including 68 cases with the conventional total abdominal hysterectomy（TAH group）,55 cases with the laparoscopic total hysterectomy（LTH group）,and 50 cases with laparoscopic modified total hysterectomy（LMTH group） were retrospectively analysed.Results： The operative time of TAH group and LMTH group were significantly less than LTH group（P 0.05）;the loss volume of blood in LMTH group was significantly less than the other groups（P 0.05）;The postoperative pain time,the recovery time of bowel peristalsis, the staying-bed time,the hospitalization time were significantly longer than the other Two groups（P 0.05）;while the rate of viginal residual stumb granuloma of LMTH group is higher than the other two groups.Conclusions： LMTH has the virtues of small trauma,the light pain,fast postoperative recovery;LMTH is worthy of clinical application and has broad application prospect as it is safe and easier to master.
