高血压脑出血周围脑组织病理改变的相关研究

摘　　要：目的探讨高血压脑出血周围脑组织病理改变。方法回顾性分析了80例高血压脑出血患者的病理资料,观察出血周围区域脑组织的病理改变,并探讨病理改变同患者基本情况(年龄、性别、出血量、高血压级别,高血压病程等)的关系。结果病理标本发现在血肿周围脑组织中大部分细小动脉有发生不同程度的透明变性;有部分细小动脉发生扩张性改变,并发现在血管周围组织中胶质细胞增生、小灶性坏死和炎性细胞浸润。在按照病理切片是否发现透明变性分组后,发现组间的患者年龄、发病时出血量及高血压病程差异有统计学意义(P<0.05);按照有无发现血管扩张性改变分组,组间的患者年龄、发病时出血量及高血压病程差异有统计学意义(P<0.05)。结论患者患高血压病年龄,病程与其脑组织中细小血管发生透明变性和血管扩张性改变相关。细小血管发生透明变性和血管扩张性改变导致血管破裂时出血量增大。Objective To investigate the pathological changes in the prehematomal brain tissues after bleeding in the hy. pertensive patients. Methods A retrospective analysis of the clinical data of 80 cases with hypertensive hemorrhage had underg- one a eraniotomy to remove the hematoma in Chengdu No. 2 People＇ s Hospital were performed to observe the pathological change in the prebematomal brain tissues and discuss the relationship between these changes and patients＇ characteristics including age, gender, volume, hypertension classification, duration Of hypertension. Results In pathological specimens slices, we found that many arterioles had been different degree of hyalinosis, some arterioles had occurred dilated change, and we also observed that neurogliocyte hyperplasia, necrosis and inflammatory cell infiltrated were appeared in some tissues around the arterioles. In respec- tively according to whether have found hyaline degeneration and whether have found arterioles dilated change divided groups, the ages, duration of hypertension and amount of bleeding were statistically significant difference between the groups（ P 〈 0. 05 ）. Con- dusion Hyaline degeneration and dilated change are produced in arterioles of hypertensive patients＇ brain tissue would be hap- pened possibly when the patient grow old or has had hypertension many years. Hyaline degeneration and dilated change would in- crease the amount of bleeding.
