活血化瘀中药对实验性肝硬化犬门静脉压力与胃粘膜血流的影响

摘　　要：目的 :研究活血化瘀中药对肝硬化门静脉压力胃粘膜血流 (GMBF)的影响。方法 :用胆管结扎法制作犬肝硬化门静脉高压模型 ,分别静滴丹参或当归注射液后测定用药前后肝硬化犬血流动力学及胃粘膜血流变化 ,并与钙通道阻滞剂硝苯吡啶进行对比研究。结果 :静滴丹参、当归后 ,肝硬化犬的门静脉压 (Ppv)、嵌塞肝静脉压 (WHVP)、肝静脉压力梯度 (HVPG)较用药前均显著下降 (P<0 .0 5或 P<0 .0 1) ,而 GMBF则显著提高(P<0 .0 5 ) ,对自由肝静脉压 (FHVP)、平均动脉压 (MAP)和心率 (HR)无显著影响 (P均 >0 .0 5 ) ,对正常犬血流动力学无显著影响。静滴硝苯吡啶后肝硬化犬的 Ppv、WHVP、GMBF、MAP、HR均较用药前显著降低 (P<0 .0 5或 P<0 .0 1) ,HVPG、FHVP无显著变化。结论 :丹参和当归等活血化瘀中药可降低肝硬化犬的门静脉压 ,与硝苯吡啶比较尚可改善胃粘膜血流 ,无动脉血压与心率的变化。Objective:To study the effects of blood activating stasis removing drugs(活血化瘀中药) on portal pressure and gastric mucosal blood flow (GMBF) of hepatocirrhosis.Methods:The models of hepatocirrhotic dogs were made by chronic bile duct ligation,the hemodynamics and GMBF in hepatocirrhotic dogs were measured before and after injection of radix salviae miltiorrhizae(丹参,RSM) and radix angelicae sinensis (当归,RAS).The effects of RSM and RAS were compared with that of calcium antagonist nifedipine.Results:After intravenous administration of RSM and RAS in cirrhotic dogs,the portal venous pressure (Ppv),wedge hepatic venous pressure (WHVP),hepatic venous pressure gradient (HVPG) were significantly decreased ( P <0 05 or P <0 01);the GMBF were significantly increased ( P <0 05),but the free hepatic venous pressure (FHVP),mean arterial pressure (MAP) and heart rate (HR) unchanged (all P >0 05).The hemodynamics in normal dogs after injection of RSM and RAS unchanged.When nifedipine was used in cirrhotic dogs,Ppv,WHVP,GMBF,MAP and HR were significantly decreased ( P <0 05 or P <0 01) with HVPG、FHVP unchanged (all P >0 05).Conclusions:Compared with nifedipine,RSM and RAS can decrease portal venous pressure and increase GMBF,MAP and HR unchange.
