氟比洛芬酯对子宫切除术患者单剂量鞘内吗啡术后镇痛效果的影响

摘　　要：目的评价氟比洛芬酯对子宫切除术患者单剂量鞘内注射吗啡术后镇痛效果的影响。方法择期拟行子宫切除术患者100例，年龄40～60岁，ASA分级I-Ⅱ级，随机分为2组（每组n=50）：吗啡组（M组）和吗啡＋氟比洛芬酯组（MF组）。所有患者均以布比卡因重比重液行蛛网膜下腔阻滞麻醉，腰麻药含布比卡因15mg＋吗啡0．2mg，调整麻醉平面至T6水平，术毕MF组静脉注射氟比洛芬酯50mg，M组静脉注射生理盐水5ml。记录术后24h镇痛效果VAS评分、舒适度（BCS）评分，镇痛期间恶心、呕吐、瘙痒、心动过缓、低血压、镇静过度和呼吸抑制等不良反应的发生情况。结果与M组相比，24h内MF组VAS评分降低，BCS评分增高（P〈0．05），术后恶心、呕吐和瘙痒的发生率差异无统计学意义（P〉0．05），2组均未发生心动过缓、低血压、过度镇静和呼吸抑制。结论氟比洛芬酯可增强子宫切除术患者单剂量鞘内吗啡术后镇痛效果，且不增加不良反应发生率。Objective To evaluate the effect of flurbiprofen on efficacy of post - operative analgesia with single - dose intrathecal morphine after abdominal hysterectomy. Methods 100 ASA Ⅰ - Ⅱ patients aged 40 -60 years under- going elective abdominal hysterectomy were randomly divided into 2 groups （ n = 50 each） ： morphine group （ M group） and morphine ＋ flurbiprofen group （MF group）. All patients were given spinal anesthesia with hyperbaric anesthetics and the cutaneous level was stabilized at T6. Bupivaeane 15 mg ＋ morphine 0. 2 mg was administered into subarachnoid space in both groups. In MF group, flurbiprofen 50 mg was administered intravenously right after the surgery, while in M group, normal saline 5 ml was administered intravenously right after the surgery. VAS score and Buggman comfort scale （BCS） score were assessed at 24 h after surgery. Post -operative complications including nausea, vomiting, bradyeardi- a, hypotension, oversedation and respiratory depression were recorded. Results Compared with M group, VAS score was significantly lower and BCS score was significantly higher in MF group （ P 〈 0. 05 ）. There was no significant differ- ence of the incidence of nausea, vomiting and pruritus among the 2 groups. No bradycardia, hypotension, oversedation or respiratory depression was observed in either group. Conclusion Flurbiprofen enhances the effect of post - operative an- algesia with single - dose intrathecal morphine after abdominal hysterectomy without adding adverse effects.
