粗针穿刺活检在乳腺癌诊断中的应用

摘　　要：目的:评价粗针穿刺活检在乳腺癌诊断中的价值。方法:对2011至2012年我院可疑乳腺癌患者301例术前进行粗针穿刺活检,所取组织石蜡切片病理检查。结果:301名可疑乳腺癌患者中,病例诊断为乳腺癌236例,术后病检均证实为乳腺癌,与粗针结果相一致,阴性65例(包括可疑病例15例),其中可疑15例中术后病检均证实为乳腺癌12例。其特异性100%,灵敏度95.16%,阳性预测值100%,阴性预测值81.53%,假阳性率0%。术后并发症:乳腺局部血肿20人,经局部加压包扎处理,血肿均缓解,无感染及种植转移、血道转移病例。结论:粗针穿刺操作简单,术后并发症少,在乳腺癌的诊断及指导治疗方面可广泛应用。Objective： To evaluate the application of coarse needle biopsy in diagnosis of breast cancer.Methods： Biopsy with large sized needle was used in 301 patients with suspected breast cancer.The obtained tissues were processed into wax slices for pathological examination.Results： Among 301 cases of suspected breast cancer,236 cases were positive,pathologically diagnosed as breast cancer,which were further confirmed by post-operative pathology.65 cases were negative,but among them,12 cases had breast cancer verified by postoperative pathology.Statistical analysis indicated that specificity was 100%,sensitivity 95.16%,positively prospective value 100%,negatively prospective value 81.53%,and pseudo positivity 0%.Post-puncture complications included local hematoma 20 cases;all were relieved with pressure bandage.No infection,no metastasis via implantation or blood flow was found.Conclusion： Puncture with large sized needle is simple,with few complications,which may be widely used in diagnosis and instructive therapy of breast cancer.
