全胰管栓塞在胰十二指肠切除术中的应用

摘　　要：自1986年9月～1989年9月,8例壶腹部周围癌患者施行了胰十二指肠切除术,并用α-氰基丙酰酸正辛酯作全胰管栓塞、不再作胰肠吻合。由于术后胰腺的腺泡萎缩,不再发生因残胰引起的胰瘘,也未发生急性胰腺炎和糖尿病。本方法简易施行,没有损伤和危险性,是防止术后胰瘘的一种较好的方法。本文还介绍了如何进行全胰管栓塞的操作体会。In this paper we present our clinical experience with the use of the pancreatic duct occlusion ( PDO ) in combination wish duodeno-pancreactomy from September 1986 to October 1989 in eight patients with carcinoma of pancieatic head or periampullary region. This procedure was to inject 4-6ml of α-cyanacytate giue into the pancreatic duct of the pancreas residual, without pancreatoenterostomy. This method is to produce pancreatic exocrine atrophy. The fact that the postoperation course was not associated with complications from the residual pancreas, was particularly striking. The advantage of this procedure is that no carbohydrate metabolism disfunction occur from the pancreas islets caused by fibrosis of residual pancreas. We conclude that pancieatic duct occlusion can prevent the occurnence of the pancreatic fistulas without any risk or injury. the procedure how to perform pancreatic duct occlusion will be described in the study.
