云南省苯丙胺类兴奋剂滥用及其与HIV/STD感染相关因素的研究

摘　　要：目的獉獉:调查云南省苯丙胺类兴奋剂滥用及与HIV/STD感染的关系。方法獉獉:采用横断面调查法,自拟《苯丙胺类兴奋剂使用情况及HIV高危行为调查问卷》。从2010年1月到2010年12月对云南省某强制隔离戒毒所近半年内入所的苯丙胺类兴奋剂滥用者进行问卷调查并采集血液标本检测HIV、梅毒和HCV。利用t检验、χ2检验比较HIV、HCV感染人群的人口学、药物滥用及高危性行为特征差异;利用Logistic model分析HIV/STD感染的可能影响因素。结果獉獉:292名苯丙胺类兴奋剂使用者中,HIV、HCV和梅毒的感染率分别为21.01%,65.07%和3.80%。使用苯丙胺类兴奋剂前后,性冲动强度有显著性差异(VAS:4.4±s 2.2 vs.7.5±s 1.8)。苯丙胺类兴奋剂使用者HIV的感染与滥用海洛因、在使用苯丙胺类兴奋剂后与陌生人发生性关系、苯丙胺类兴奋剂作用下使用安全套情况有关;而HCV感染与职业、多药滥用、滥用海洛因、在使用苯丙胺类兴奋后与陌生人发生性关系有关。结论獉獉:苯丙胺类兴奋剂使用者HIV与HCV感染与高危性行为有关,且以HIV尤为显著。使用苯丙胺类兴奋剂会增强性冲动从而发生性行为,且多为群体性性行为,使用后安全套的使用情况较差,这些是该人群HIV/STD传播的重要因素。Objective：To explore the current situation of amphetamine-type stimulants（ATS） abuse and the HIV/STD infection,and to analyse the associate factors between ATS abuse and HIV/STD infection.Methods：Two hundred and ninety two participants from a certain drug rehabilitation center in Yunnan province from January 9 to December 29 in 2010 were interviewed,completed a questionnaire,and were screened with the blood sample.Descriptive statistics,t test,the χ2 square tests and binary Logistic regression models were used to analyze the differences of HIV/STD among different subpopulations.Results：The prevalence of HIV,HCV and syphilis were 21.01%,65.07% and 3.80%,respectively.A significant difference was found in sex impulse before and during ATS abuse.HIV infection was related with heroin abuse,having sex with strangers after ATS abuse and condom usage after ATS abuse,and HCV infection was related with occupation,poly-drug abuse,heroin abuse and having sex with strangers after ATS abuse.Conclusion：ATS increase the sex impulse and lead to the sex behaviors that are mostly group sex and poor condom use,which make the ATS abusers a high risk group of HIV/STD infection.
