摘  要:目的探讨肾素血管紧张素系统(RAS)阻滞剂对IgA肾病(IgAN)患者血和尿转化生长因子1(TGF-1)的影响及其意义。方法比较12例长期服用小剂量血管紧张素转化酶抑制剂(ACEI)或血管紧张素Ⅱ受体阻滞剂(ARB)的IgAN患者和11例均未服用上述药物的患者治疗前后血和尿的TGF-1及临床参数的变化。结果 12例IgAN患者在长期服用RAS阻滞剂后其尿TGF-1和8 h尿蛋白均较治疗前降低,未经上述药物治疗的11例IgAN患者的尿TGF-1及8h尿蛋白均较治疗前明显上升。结论使用小剂量的RAS阻滞剂可明显降低IgAN尿TGF-1的水平,但不能将其降至正常范围,且不能完全阻断疾病的进展。Objective To investigate the influencing of renin-angiotensin system(RAS) on changes of blood and urine transforming growth factor-1(TGF-β1) in IgA nephropathy patients and its significance.Methods Twentythree patients with IgA nephropathy patients were enrolled in this study;among which,12 cases were with long-term use small doses of angiotensin 1-converting enzyme inhibitors(ACEI) or Ⅱ angiotensin receptor blockers(ARB),and the other 11 cases did not take the drugs.Enzyme-linked immunosorbent assay(ELISA) to measure blood and urine TGF-β1 before and after treatment and clinical parameters were examined.Results tUrine TGF-β1 and 8 h urine protein level were significantly decreased at the end of the observation period among the 12 patients who took the drugs;and urine TGF-1 and 8h urine protein level were increased dramatically in 11 cases who did not take drugs.Conclusions Small doses of RAS blockers can obviously reduce IgA nephropathy urine TGF-β1 level,but can't bring it down to normal range,and can't completely blocked the progress of the disease.
