自拟中药方剂延玉合剂治疗老年稳定劳力型心绞痛的临床观察

摘　　要：目的探讨自拟中药方剂延玉合剂（以下简称合剂）对中医辨证属胸痹标实证的老年稳定劳力型心绞痛的临床疗效。方法将中医辨证属胸痹标实证的556例年龄〉160岁的稳定劳力型心绞痛患者按脉证表现分为气滞、寒凝、痰浊和血瘀证四型，每型随机分为治疗组、对照组。两组均给予冠心病2级预防治疗，治疗组另给予合剂，30d为一疗程。结果治疗组在减少心绞痛发作次数、降低心绞痛等级、改善心功能及降低心率、血压、血脂、血糖、血黏度、炎症因子等方面疗效优于对照组（P〈0．05），预后也优于对照组。治疗组以气滞、寒凝型疗效最好，痰浊、血瘀型稍差（P〈0．05）。结论合剂治疗胸痹标实证老年稳定劳力型心绞痛疗效确切，气滞、寒凝型疗效更佳。Objective To explore the clinical effect of Jia Wei Stigma Maydis decoctions on the patients with stable angina pectoris belonged to thoracic obstruction standard asthenia syndrome of traditional Chinese medicine syndrome differentiation. Methods 556 patients with stable angina pectoris belonged to traditional Chinese medicine syndrome -type in thoracic obstruction standard asthenia syndrome were divided into four syndrome - type of qi stagnation, cold coagulation, phlegm, blood stasis according to syndrome of sphygmopalpation, and the patients in every syndrome - type were randomly divided into treatment group and control group. All the groups were given second grade preventive therapy of coronary artery disease. The treatment group was additionally given Stigma Maydis decoctions, 100 mL, three times each day. The treatment duration was three months. Result The treatment group had significant effect on reducing the frequency of angina attack, lowering the level of angina and reducing blood lipids, blood glucose, blood viscosity, inflammatory cytokines, all of these was better in the treatment group than in control group. The prognosis was better in treatment group than in control group. Curative effects were compared between four syndrome - types in treatment group, the effects in qi stagnation group and cold coagulation group were best, and which were less in phlegm and blood stasis group（ P 〈0.05 ）. Conclusion Stigma Maydis decoctions have certain effect for treating stable angina pectoris belonged to thoracic obstruction standard asthenia syndrome. The effects in group of qi stagnation and cold coagulation were better than those in phlegm and blood stasis group. Prognosis of treatment group was better than that of control group.
