子宫静脉内平滑肌瘤病8例临床分析

摘　　要：目的探讨子宫静脉内平滑肌瘤病的临床表现、病理特点及诊治要点。方法回顾性分析厦门大学附属中山医院在2002年7月至2010年12月收治的8例子宫静脉内平滑肌瘤病患者的临床资料。结果 8例患者6例术前未诊断,全部接受手术治疗,术后病理均证实为子宫静脉内平滑肌瘤病。8例中1例病变已经累及至心脏;另1例累及至右髂内静脉、右卵巢静脉,术后4年发现盆腹腔包块,外院再次手术病理提示为腹膜播散性平滑肌瘤病;其余6例病变均累及至宫旁阔韧带内子宫静脉。1例术后可疑复发,肿块较小随访中。结论提高超声医师和妇科医师对子宫静脉内平滑肌瘤病的认识,有利于对该病的术前和术中诊断。Objective To investigate clinical feature,pathologic characteristics and course of diagnosis and treatment of in- travenous leiomyomatosis （IVL） and sum up experience and improve the level of diagnosis and treatment. Methods 8 casesof IVL were retrospected,which had been treated and had been diagnosed by pathologic examination in Xiamen University Affilicated Hospital of Zhongshan,between July,2002 and Dec,2010. Results Six patients of 8 cases which were treated surgically and were confirmed suffering from intra-venous leiomyomatosis （IVL） by pathology didnt be diagnosed before operation. There was one patient in which IVL was involved in heart. Another patient,in which IVL was involved in right internal iliae vein and fight ovarian vein, suffered from abdominal masses 4 years after first operation and had another operation in other hospital and was diagnosed disseminated peritoneal leiomyomatosis. The lesions of other 6 pa- tients were involved in uterine vein inside the broad ligament. One of them was suspiciously relapsed with a small lesion af- ter operation and followed up. Conclusion Improving ultrasound physicians and gynecologists＇ understandings of intra-ve- nous leiomyomatosis （IVL） is helpful to the diagnosis of IVL before operation and during operation.
