中西医结合治疗桡骨远端骨折研究述评

摘　　要：目的:探讨中西医结合治疗桡骨远端骨折的研究情况。方法:总结、归纳和分析近年来的相关文献,对桡骨远端的分类方法、影像学检查、治疗原则、治疗方法的目前研究状况做一概述。结果:桡骨远端骨折的分类方法多种多样,例如Older分类法、Fryk-man分类法、AO/ASIF分类系统、Melone分类法、Missakian分类法、Fernandez分类法、Gartland-Sarmiento分类法、"齐藤"分类法、Doi分类法等,桡骨远端骨折的治疗原则是恢复腕关节正常的掌倾角,尺偏角,恢复桡骨的长度(及恢复尺桡骨茎突正常解剖结构),并恢复腕关节的正常解剖结构,桡骨远端关节面的平整,在此基础上,选择正确相对稳定的内、外固定法固定骨折端,在稳定牢靠固定的基础上,是患肢早期的功能锻炼,早期促进骨折愈合。治疗方法包括中医辨证论治、传统保守治疗、经皮穿针术、外固支架技术、切开复位内固定术、腕关节镜指引下复位、自体骨或骨替代物移植、人工腕关节置换。结论:桡骨远端骨折的治疗方法多样,疗效显著。Objective： To investigate research progress of therapeutic method of combination of traditional Chinese and western medicine treatment of distal radius fractures.Methods： The related literatures in recent years were summarized,concluded and analyzed,and current research situations of classification method,imaging examination,treatment principle and treatment methods of distal radius were overviewed.Results： The classification methods of distal radius fractures were various,such as Older classification,Fryk-man classification,AO / ASIF classification system,Melone classification,Missakian classification,Fernandez classification,Gartland-Sarmiento classification,Qi Teng classification,Doi classification,etc.The treatment principle of distal radius fractures was to restore the normal palmar tilt and radial inclination of wrist joints,and to restore the length of the radius,and recovery the normal anatomic structures of the ulnar radial styloid,and to recover the normal anatomic structure of wrist joints,the flatness of the articular surface for distal radius,and on the basis of the above,exact and relatively stable internal and external fixation were selected to fix the fracture site.And early-stage functional exercise for affected limb was done to promote fracture healing based on stable and sturdy fixation.Syndrome differentiation and treatment of traditional Chinese medicine,traditional conservative therapy,percutaneous needling technique,external fixation supporter technique,open reduction and internal fixation,wrist arthroscope guidance reset,autologous bone or bone graft substitutes,and artificial wrist joint replacement were used to treat the disease.Conclusion： Treatment methods of distal radius fractures are various,and the curative effect is distinct.
