关元穴灸法治疗腰椎间盘突出症的临床功能与电生理评价

摘　　要：目的:采用VAS量表、JOA下腰痛评估及肌电图监测技术探讨关元穴灸法治疗腰椎间盘突出症在临床功能、电生理变化上影响。方法:将60例患者按随机数字表法分为艾灸组、针刺组各30例。艾灸组使用灸架艾条温和灸关元,距皮肤2 cm,每次1 h,每天1次;针刺组取腰椎相应的华佗夹脊穴,肾俞、大肠俞、环跳、秩边、委中、阳陵泉、绝骨,每次30 min,两组均以治疗3周为1个疗程。在治疗前后两组患者分别进行肌电图检查并采用JOA量表及VAS量表进行评分。结果:治疗后两组VAS和JOA评分比较,差异有统计学意义(P<0.05),治疗后胫骨前肌或腓肠肌内侧头正相波、失神经电位、神经波幅比较,差异无统计学意义(P>0.05)。结论:关元灸法治疗效果优于针刺疗法,且艾灸治疗本身具有无创痛、简便易行、无不良反应等诸多优点。Objective： The VAS scale,JOA low back pain assessment and EMG monitoring technology were applied to explore the effects of Clinical function and electrical physiological evaluation on the treatment of protrasion of lumbar intervertebral disc herniation by moxibustion of Guanyuan point.60 cases were divided into the moxibustion group（30 cases） and the acupuncture group（30 cases）.The moxibustion group was given gentle moxibustion at guanyuan point in moxa stick by moxibustion frame,from the skin 2 cm,1 h each time,once a day.The acupuncture group selected acu-points of lumbar corresponding huatuo jiaji,shenshu,dachangshu,huantiao,zhibian,weizhong,yanglingquan,juegu,30min each time.Both groups regarded three weeks as a course of treatment.The patients were examined by EMG and graded by JOA scale and VAS scale before and after the treatment.Results： It had difference between the two groups after treatment in VAS and JOA score（P〈0.05）.It had no difference of the tibialis anterior muscle or the medial head of the gastrocnemius positive phase wave,loss of nerve potential and nerve amplitude after the treatment（P〈0.05）.Conclusion： The therapy of moxibustion of Guanyuan point is superior to the acupuncture,and the therapy of moxibustion has many advantages,such as no trauma and no adverse reaction,which is simple and easy for patients to accept,and so on.
