摘  要:目的探讨系统性红斑狼疮(SLE)患者尿中N-乙酰-β-氨基葡萄糖苷酶(NAG)水平及其临床意义。方法根据尿微量蛋白结果将65例SLE患者分为单纯肾小管受累组(n=30)和肾小管与肾小球均受累组(n=35)。21例健康体检者作为对照组。散射比浊法和比色法检测尿中NAG水平。结果组间比较显示:单纯肾小管受累组和肾小管与肾小球均受累组的尿NAG水平均显著高于对照组(P<0.05),而单纯肾小管受累组和肾小管与肾小球均受累组的尿NAG水平无明显差异(P>0.05)。65例SLE患者尿NAG与尿β2-微球蛋白呈显著的正相关性(r=0.96,P<0.05)。结论 SLE患者的尿检结果提示肾小管的病变可单独存在,而肾小球病变与肾小管病变同时存在。狼疮肾炎患者尿NAG水平显著增高,且与肾小管病变密切相关。Objective To explore the level of N-acetyl-β-glucosaminidase (NAG) and its clinical significance in patients with systemic lu- pus erythemalosus (SLE). Methods According to the results of urine micro-protein, sixty-five patients with SLE were reeruited and divided into simple tubular involvement group (n =30) and renal tubular as well as glomerular involvement group (n =35 ). There was no patient with simple glomerular involvement. Twenty-one age- and sex-matehed healthy individuals were enrolled as eontvol. NAG was deteeted by seattering turbidimetry and colorimetry. Results The levels of urinary NAG were significantly higher in the simple tubular involvement group and renal tubular as well as glomerular involvement group compared with the control group ( P 〈 0.05 ). There was no significant dif- ference between the simple tubular involvement group and renal tubular as well as glomerular involvement. Positive correlation was found be- tween urinary NAG and urinary β2-micro-globulin in 65 patients (r =0.96, P 〈 0.01 ). Conclusion Renal tubular lesions can exist alone in patients with lupus nephritis. Urinary NAG is a useful marker for the detection of renal involvement in lupus nephritis.
