温补肾阳法对肾阳虚变应性鼻炎大鼠模型鼻黏膜重塑的影响

摘　　要：目的:研究温补肾阳法对肾阳虚变应性鼻炎(AR)大鼠模型鼻黏膜重塑的影响。方法:在腺嘌呤致肾阳虚病理模型的基础上,用卵清蛋白为过敏原对大鼠致敏,建立肾阳虚变应性鼻炎大鼠模型。以温补肾阳代表方剂金匮肾气丸灌胃,鼻中隔黏膜组织标本石蜡包埋,采用苏木素-伊红(HE)、阿辛蓝-过碘酸-希夫(AB-PAS)和Masson三色胶原染色(MT)法染色,计数鼻中隔黏膜中嗜酸粒细胞、上皮杯状细胞数量、半定量测量鼻中隔黏膜和鼻甲基底膜区及上皮下胶原含量。结果:病理结果显示温补肾阳组鼻黏膜嗜酸粒细胞数、杯状细胞化生数、MT染色蓝色面积呈逐渐减少趋势,但仍明显高于正常对照组(P<0.05)。结论:AR鼻黏膜存在重塑,且随着变应原的持续接触,重塑将进一步加重,最终发生不可逆性改变。金匮肾气丸可有效控制AR,减缓或一定程度逆转鼻黏膜重塑,而对于已发生的不可逆性改变并无明显逆转或修复作用。Objective:To observe the effect of kidney-yang warming therapy on nasal mucosal remodeling of allergic rhinitis(AR) animal model.Methods:Kidney-yang-deficiency rat models were established first by adenine inducing kidney yang and then by sensitization with inhalation of ovalbumin.The model rat was given gastric infusion of the solution of Jinkui Shenqi Pills.After treatment,tissue sample of nasal septum was embedded with paraffin,and then the paraffin sections were stained with hematoxylin and eosin(HE),alcian blue-periodic acid-Schiff(AB-PAS),and Masson's Trichrome(MT).The eosinophil and epithelial beaker cell count in nasal septal mucosa was examined,and the collagen content was determined by semi-quantitation method in the basement membrane zone and epithelium of nasal septal mucosa and nasal turbinate.Results:The results of pathological examination showed a decreasing trend in the eosinophil and epithelial beaker cell count and in the MT-staining blue area of Jinkui Shenqi Pills group,but the indexes were still higher than those in the normal model group(P < 0.05).Conclusion:Remodeling occurs in the nasal mucosa of AR rats,and aggravates after the persistent exposure to the allergen,and finally becomes irreversible.Jinkui Shenqi Pills,a representative formula of kidney-yang warming therapy,can relieve the clinical symptoms of AR and slow down the remodeling of AR,but has no effect of the irreversible changes.
