摘 要:目的探讨不同糖耐量人群胰腺脂质沉积与IR的关系。方法选择新诊断T2DM患者46例,IGR 37例及健康对照(NC)者37名,接受上腹部CT平扫,用脾胰CT差值评估胰腺脂质沉积程度,计算稳态模式评估胰岛素抵抗指数(HOMA-IR)。结果脾胰CT差值在NC、IGR和T2DM组内[(5.4±6.0)Hu、(8.2±6.1)Hu、(15.5±7.0)Hu]递增,HOMA-IR也逐渐增加;多元线性回归分析表明,脾胰CT差值是HOMA-IR的独立相关因素(P<0.01)。结论胰腺脂质沉积与IR相关,可能参与T2DM的发生发展。Objective To investigate the association between the degree of pancreatic lipid deposition and in IR in people with different glucose tolerances. Methods The 46 newly diagnosed T2DM patients, 37 cases with IGR, and 37 healthy subjects NC were selected (aged 67. 1-t-6.8 years in average) for study. All subjects received upper abdominal CT scan and their Hounsfield units (Hu) were measured. Homeostasis model insulin resistance index (HOMA-IR) was calculated to evaluate the degree of their insulin resistance. Results The degree of pancreatic fat deposition in the NC, IGT, and T2DM groups increased progressively [- (5.4-4- 6.0) Hu , (8. 2 ~ 6.1) Hu , ( 15.5 -4- 7. O) Hu ) ], the same trend occurred in HOMA-IR. Multiple regression analysis showed that the degree of pancreatic fat deposition was an independent relevant factor of HOMA-IR index (P〈0. 01). Conclusion The pancreatic fat deposition is closely related to insulin resistance and may be involved in the development of T2DM.
