摘  要:目的探讨宿迁医改十年后影响居民就医行为与态度的主要因素。方法 2011年7月13—21日对宿迁市三区多阶段分层随机抽样进行现况研究。结果 79.7%(441/553)的宿迁居民不满意现行的医疗制度。目前居民年人均医疗费用支出为278.6元,单因素分析显示对现行医疗制度是否满意的居民在就医费用来源,选择医生、医院的注重因素,收费合理性并能否承担,护士、医生的态度改善否,对医院设施环境、医生诊疗水平满意否等方面差异均有统计学意义(P<0.05);多因素分析显示家庭月均收入低、医疗费用自费、自己以往就医成功的经验、收费不合理和医生的态度未改善是影响居民满意度的主要因素(P<0.05)。结论医改后宿迁居民的就医满意度不高,还需在提高经济收入、加强医生管理等方面做好准备来提高改革的成效。Objective To explore the influencing factors of healthcare-seeking behavior and attitude among the residents in Suqian after healthcare system reform(HSR). Methods The residents in one district of Suqian were selected using multi-stage stratified random sampling and a cross-sectional survey was conducted. Results About 79.7%(441/553) of the residents were not satisfied with the existing health care system of Suqian.Currently the residents′ annual medical spending was an average of 278.6 yuan per capita.Single-factor analysis showed that the residents satisfied with the current HSR and those dissatisfied differed significantly in the following aspects:source of medical spending,hospital and doctor selection criteria,reasonableness and affordability of the charges,satisfaction with the medical staff′s attitudes,satisfaction with the healthcare environment and facilities,and the doctors′ medical competency(P0.05).Multivariate analysis showed the reasons for residents′ dissatisfaction:low monthly household income,high healthcare costs paid by residents themselves,past experience of successful medical treatment,the unreasonable charges,and the unimproved attitudes of the doctors(P0.05). Conclusion Most of the residents are not satisfied with the medical reform of Suqian.In order to enhance the effect of the reform,efforts should be made to improve the economic income and to strengthen management of the doctors.
