摘  要:目的研究降糖调脂灵对2型糖尿病(T2DM)大鼠大血管病变的保护作用,并探讨其可能机制。方法将40只健康成年雄性Wistar大鼠随机分为4组:对照组、模型组、双胍组、中药组,各10只。对照组大鼠喂以标准饲料且不做任何处理;其余各组大鼠通过高糖高脂饲料喂养+腹腔注射链脲佐菌素(STZ)诱发糖尿病,继续喂以高糖高脂饲料。同时双胍组大鼠给予二甲双胍134 mg.kg-1.d-1灌胃,中药组大鼠给予降糖调脂灵690 mg.kg-1.d-1灌胃;而对照组和模型组给予相当量的0.9%氯化钠注射液灌胃。3个月后检测各组大鼠血糖、糖化血红蛋白(HbA1c)、胰岛素、血脂〔总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)〕水平;计算胰岛素敏感性指数(ISI);酶联免疫吸附(ELISA)法检测血清可溶性转铁蛋白受体(sTfR)水平;RT-PCR法检测平滑肌细胞转铁蛋白受体(TfR)mRNA表达量;透射电镜观察主动脉平滑肌细胞超微结构改变。结果 (1)4组大鼠血糖、HbA1c、胰岛素、ISI、血脂、sTfR水平及主动脉壁平滑肌细胞TfR mRNA表达量比较,差异均有统计学意义(P<0.05);其中模型组大鼠血糖、HbA1c、胰岛素、TC、TG、LDL、sTfR水平及TfR mRNA表达量高于对照组,ISI及HDL水平低于对照组,差异有统计学意义(P<0.05);双胍组大鼠胰岛素、血脂、sTfR水平及mRNA表达量与模型组比较,差异均无统计学意义(P>0.05);而中药组大鼠血糖、HbA1c、胰岛素、TC、TG、LDL、sTfR水平及TfRmRNA表达量均低于模型组,ISI及HDL水平高于模型组,差异有统计学意义(P<0.05)。(2)模型组大鼠可见主动脉壁平滑肌细胞成退行性改变,细胞核内电子密度升高,异染色质增多,细胞质内大量线粒体出现明显的嵴断裂、空泡样变、髓鞘样变,高尔基复合体内出现大空泡。双胍组大鼠主动脉壁平滑肌细胞也出现类似变化。中药组大鼠主动脉壁平滑肌细胞损坏轻微,但也有少量Objective To study the protective effect of Jiangtangtiaozhiling in T2DM angiopathy and to investigate possible mechanism. Methods 40 healthy Wistar rats were randomly divided into control group,model group,metformin group and TCM group with each group 10 rats.The control group was given standard diet,while other groups were given high glucose and fat diet+ intraperitoneal injection of STZ to induce diabetes.The metformin group was given intragastric administration of 134 mg·kg-1·d-1 metformin and the TCM group was given intragastric administration of 690 mg·kg-1·d-1 Jiangtangtiaozhiling.The control group and model group were both given intragastric administration of 0.9% sodium chloride injection in the same volume.After three months treatment,blood glucose,HbA1c,insulin,blood lipids(TC,TG,HDL and LDL) of each group were detected and ISI was calculated.ELISA method was used to detect sTfR,RT-PCR method was used to detect TfR mRNA expression and transmission electron microscope was used to observe the cell ultrastructure changes of artery smooth muscle. Results(1) The blood glucose,HbA1c,insulin,ISI,blood lipids,sTfR and TfR mRNA expression in artery smooth muscle in the four groups showed statistically significant differences(P0.05).The blood glucose,HbA1c,insulin,TC,TG,LDL,sTfR and TfR mRNA expression in the model group were significantly higher than in the control group,but the ISI and HDL were significantly lower than the control group(P0.05).The insulin,bolld lipods,sTfR and it′s mRNA between the metformin group and model group showed no statistically significant difference(P0.05).The blood glucose,HbA1c,insulin,TC,TG,LDL,sTfR and TfR mRNA expression in the TCM group were significantly lower than in the model group,but the ISI and HDL were significantly higher than the model group(P0.05).(2) Electron microscope observation showed that there were retrograde changes in smooth muscle cells in the rats′ aorta wall in model group,and the electronic density and heterochromatin
