重庆市永川区男男性行为人群HIV感染率及其影响因素分析

摘　　要：目的了解男男性行为人群(Men who have sex withm en,MSM)HIV感染率及其影响因素,为开展有针对性的艾滋病防治工作提供建议。方法通过滚雪球的方法招募MSM人群在知情同意的情况下进行面对面的问卷调查和抽取血样检测HIV与梅毒,用SPSS12.0对调查资料进行描述性分析,HIV感染率影响因素采用多因素Logistic回归分析进行鉴别。结果共计调查218名MSM人群,艾滋病知识知晓率37.6%,最近1次与男性发生肛交时安全套使用率为42.2%,最近6个月与同性发生肛交时每次都使用安全套的比例占3.2%,HIV感染率为16.5%,梅毒感染率为15.1%。艾滋病防治知识知晓率(OR=2.90,95CI:1.01～8.34)、第1次与男性发生肛交时是否使用安全套(OR=4.98,95CI:1.61～15.36)、过去1年是否出现性病相关症状(OR=4.89,95CI:2.13～11.14)是HIV感染率的独立影响因素。结论永川区MSM人群HIV感染率高,艾滋病防治知识知晓率低,安全套使用率低,艾滋病防治形势严峻。OBJECTIVE To understand HIV prevalence and its associated factors to provide the suggestion for AIDS prevention and control among men who have sex with men. METHODS MSM recruited through snowballing were investigated by face to face with their consent. Blood samples taken were tested for HIV and syphilis. The data were analyzed by SPSS12.0. The influ- encing factors were identified by muhi-variate logistic regression. RESULTS A total of 218 MSM were investigated. The rate of awareness about AIDS was 37.6%. The rate of condom use during anal sex with men in the last time and in the past six months was 42.2% and 3.2% respectively. The prevalence of HIV and syphilis was 16.5% and 15.1% respectively. The rate of awareness about AIDS （OR = 2.90, 95CI： 1.01-8.34）, the rate of condom use during sex with man in the first time （OR = 4.98, 95CI： 1.61-15.36） and the history of STD symptom in the past year （OR = 4.89, 95CI： 2.13-11.14） were independent factors of HIV prevalence. CONCLUSION The prevalence of HIV and syphilis is high among MSM. The knowledge about AIDS is very poor and the rate of condom use is very low. AIDS epidemic among MSM is very serious in Yongchuan district.
