深圳市2010年感染HIV的男男性接触者原发耐药基因突变监测

摘　　要：目的了解深圳市2010年男男性接触者(MSM)HIV-1耐药传播情况,监测2010年该地区MSM人群HIV毒株的流行情况。方法收集深圳市2010年专题调查的14例HIV-1抗体阳性的MSM血浆,提取病毒RNA,采用RT-PCR方法扩增pol区的基因片断,并对扩增片断进行序列测定和分析。结果 CRF01_AE亚型占45.39%(64/141),CRF07_BC亚型占39.71%(56/141),B亚型占14.89%(21/141)。3份出现NRTI耐药突变,突变的类型分别为:L210W,T215S,T215I;1份出现NNRTI耐药突变,突变的类型为:K103N,Y181C,P225H;无样本出现PI耐药突变,总体耐药突变率为2.8%。结论深圳地区MSM人群中耐药传播目前仍处于低度水平。感染株主要为CRF01_AE、CRF07_BC和B3种亚型,CRF01_AE仍是深圳市MSM人群中HIV主要流行亚型,CRF07_BC重组亚型在深圳市MSM人群中增长迅速。OBJECTIVE To investigate the transmission of drug resistant human immunodeficiency virus 1 （HIV-1） infection among men who have sex with men （MSM） in Shenzhen and identify the subtype distribution of HIV in this region during 2010. METHODS 141 plasma samples were collected from HIV-1 infected patients who were identified in a specific MSM survey in 2010. Viral RNA were isolated, pol regions were amplified by RT-PCR, and sequenced. RESULTS There were three HIV-1 subtypes including CRF01_AE, CRF07_BC and B among the 141 MSMs. Genotyping showed that 45.39% （64/141） were CRF01 AE, 39.71% （56/141） were CRF07_BC and 14.89% （21/141） were subtype B. Drug resistant mutations to nucleo- side reverse transcriptase inhibitors （NRTIs） were found in three of the 141 MSMs, as L210W, T215S and T215I. One patient was detected holding non-nucleoside reverse transcriptase inhibitors （NNRTIs） -related drug resistant mutations, in K103N, Y181C and P225H. No case showed protease inhibitors （PIs） -related mutation. The primary drug resistant rate was 2.8%. CONCLUSION The rate of drug resistance transmission among MSM in Shenzhen is still low in 2010. There are three HIV-1 subtypes among MSMs in Shenzhen and the CRF01_AE is the most prevalent HIV-1 subtype as before, the CRF07_BC is in- creasing fast among MSMs.
