综合ICU机械通气患者床头抬高现状分析

摘　　要：目的了解目前综合ICU机械通气患者床头抬高30°～45°的实施现状。方法成立调查小组,选择2011年1～3月某院综合ICU的268例有创机械通气患者为研究对象,从6个时间点:8:00、12:00、16:00、20:00、24:00、4:00分别测量床头抬高角度,并研究主管护士的职称、工作年限等对床头抬高合格率的影响。结果床头抬高≥30°仅为13.9%,而抬高角度在15°﹤30°为76.0%;各时间点的合格率差异有统计学意义(P﹤0.05);护士的职称和工作年限与床头抬高合格率无明显关系。结论综合ICU机械通气患者半卧位目前没有广泛实施或实施角度不够,应采取措施提高床头抬高的临床依从性,提高和改善危重病人的监护治疗质量和安全。OBJECTIVE To investigate the present condition of head of bed elevation 30°-45°of patients who cured with me- chanical ventilation in comprehensive ICU. METHODS The investigation team was founded, 268 patients cured with mechani- cal ventilation in comprehensive ICU from January 2011 to March 2011 were investigated, then measured the angle of head of bed elevation from six points of time： 8： 00, 12： 00, 16： 00, 20： 00, 24： 00, 4： 00, and researched the influence of the positional titles and working years of nurses on the passing rate of head of bed elevation. RESULTS The patients above 30 were only 13.9%, but the patients whose angle of head of bed elevation between 15°and 30° were up to 76.0% ; The passing rate of the six points of time was obviously different （P 〈 0.05）. The positional titles and working years of nurse had no obvious effect on the passing rete of head of bed elevation. CONCLUSION The semireclining position of patients who cured with me- chanical ventilation in comprehensive ICU is no widely implemented or the angle is not enough big at the moment, we should adopt an important measure to raise the clinical compliance of head of bed elevation, then we can improve the quality and safety of treatment.
