超声心动图在评估肥厚型梗阻性心肌病右室功能中的应用

摘　　要：目的探讨超声心动图Tei指数评价肥厚型梗阻性心肌病患者右心功能的临床价值。方法经三尖瓣血流和肺动脉瓣血流,应用脉冲多普勒技术对52例肥厚型梗阻性心肌病患者(HOCM组)和52例对照组人群标准心尖四腔心切面及主动脉短轴切面分析,计算Tei指数。结果与对照组相比,肥厚型梗阻性心肌病患者Tei指数显著增大(P<0.001),ICT延长(P<0.001),IRT延长(P=0.001),RVET明显缩短(P<0.001)。结论 Tei指数能简便、敏感、综合评价肥厚型梗阻性心肌病右心室整体收缩舒张功能。Objective To investigate the value of Tei index for estimating right ventricular globalfunction in patients with dissymmetry hypertrophic obstructive cardiomyopathy. Methods Pulsed wave doppler of transtricuspid flow and transpulmonic valve flow were performed in hypertrophic obstructive cardiomyopathy patients （HOCM group, n = 52） and healthy control subjects （n = 52 ） at apical four-chamber view and short-axis view of aortic root, and Tei index was calculated. Results Tei index was significantly different between patients with HOCM and normal subjects （P 〈 0.001）. Compared with normal subjects, ICT and IRT in HOCM group were remarkably prolonged and RVET was sig- nificantly shortened （P 〈 0.001）. Conclusion For the patients of HOCM, Tei index is valuable and simple for evaluat- ing the systolic and diastolic functions of right ventricular with high sensitivity.
