摘 要:目的:观察和比较消瘀散新旧剂型(涂膜剂、即型凝胶剂和散剂)局部外敷对急性软组织挫伤新西兰大白兔模型的治疗效果和透皮性能,选择优势剂型,并初步探讨其促进急性软组织挫伤修复的机制。方法:将60只新西兰大白兔造成急性软组织挫伤模型后,随机分为散剂组(A)、涂膜剂组(B)、即型凝胶剂组(C)、模型组(D),A、B组及C组分别用消瘀散涂膜剂、即型凝胶剂和散剂治疗,D组予空白贴剂外敷。各组用药后第2、4、6 d作大体观察评分;每组每个时相点取5只实验大白兔,按照要求处理标本并HE染色光镜下观察组织形态学变化;以有效成分大黄素累积透皮百分率为测定指标,高效液相色谱法(HPLC)测定A、B组及C组进行体外透皮实验,用改良的Franz扩散池及离体兔皮进行体外透皮实验,比较各种剂型的透皮速率、透皮百分率与皮肤储量的差别。结果:大体观察评分,C组与A组第4、6 d比较,B组与A组第6 d比较,症状有明显改善(P<0.01);组织病理切片HE染色可见,第6 d组织学观察评分C组指标优于A组(P<0.05);各种剂型间体外透皮实验结果显示:即型凝胶剂透皮速度最快,皮肤储量最多,涂膜剂透皮总量最多。结论:消瘀散外敷治疗急性软组织挫伤新西兰大白兔模型,新剂型的吸收和疗效优于旧剂型,而两种新剂型间的比较无显著性差(P>0.05),即型凝胶剂在具体计分上总体略优于涂膜剂。Objective:To observe and compare the therapeutic efficacy and the transdermal performance of old and new formulations Xiaoyusan(Xiaoyu plastic,Xiaoyu In-stiu gel and Xiaoyusan)in external use of treating New Zealand White rabbit models with acute injury of soft tissue,to select the advantage formulation and evaluate its promotion of the mechanism of acute soft tissue injury repair.Methods:60 New Zealand White rabbits were attacked to obtain animal models of acute soft tissue injure,and randomly divided into 4 groups:A.powder group,treated with old formulation;B.plastic group,treated with the new formulation;C.In -situ gel group,treated with the same as B;D.model group,treated with empty one.Each group was treated respectively with different formulations of Xiaoyusan for 2,4 and 6 days,general observations were taken and scored;HE staining was used for histological observation under light microscope;plasma concerntration(emodin) was determined by the high performance liquid chromatographic(HPLC);group A,B and group C in vitro penetration test in rabbit skin and the Franz diffusion cell were analyzed and compared.Results:There was difference in general observation comparing between group C and group A in day 4,group C and group B in day 6(P0.01);HE staining showed that group D excelled group C in day 6 in grades of pathological tissue section(P0.05).In vitro penetration test illustrates,the fastest of penetration rate and the most of the reserves in skin is In-situ gel,the most of the total amount is plastic.Conclusion: New formulation of Xiaoyusan in the external use of treating the New Zealand White rabbit models with acute soft tissue injury was better than old formulation in absorbability and curative effect.Comparison between two new foumulations has no statistically difference(P0.05),but In -situ group is better than plastic group in specific points.
