剖宫产术后子宫瘢痕妊娠临床分析

摘　　要：目的探讨剖宫产术后子宫瘢痕妊娠的诊断与治疗方法,观察其临床效果。方法选择2011年12月～2012年6月本院收治的剖宫产术后子宫瘢痕患者16例,对其诊断、治疗方法及治疗结果等资料进行回顾性分析。结果 12例患者治疗前经超声检查得到明确诊断。9例行子宫动脉动脉栓塞加人工流产术治疗成功;3例单纯行甲氨蝶呤保守治疗但效果不理想;4例腹腔镜下行宫腔镜病灶清除术治疗成功,无全子宫切除病例。结论早期准确诊断对剖宫产术后子宫瘢痕妊娠具有重要的意义。超声是诊断剖宫产术后子宫瘢痕妊娠简便而可靠的方法。保守治疗和手术治疗是可选择的安全有效的治疗方法,Objective To explore the diagnosis and treatment of cesarean scar pregnancy （CSP）, and observe its clinical effect. Methods From December 2011 to June 2012, 16 cases of cesarean scar pregnancy in our hospital were chosen. Their detailed clinical data, diagnosis and treatment methods were retrospectively analyzed. Results Twelve cases were made a definite diagnosis by sonograph before treatment. 9 cases were successfully treated with uterine artery embolization; 3 cases were treated with methotrexate only, but the effect was not ideal. 4 cases were successfully treated with uteroscope resection under laparoscope. No cases were treated with complete hysterectomy. Conclusion Early accurate diagnosis has important significance for CSP. Sonography is a convenient and reliable diagnositic method for CSP. Conservative treatment and surgical treatment are optional safe and effective treatment for CSP.
