电针耳穴与电针体穴治疗胆绞痛疗效比较

摘　　要：目的观察并比较电针耳穴和电针体穴治疗胆绞痛产生疗效时间的差异来研究针刺耳穴和针刺体穴不同的机制。方法耳穴组26例,体穴组24例。耳穴取右侧胰胆区和耳迷根,体穴取右胆俞和胆区压痛处,两组电针方法相同。观察电针开始后1h和2h的疗效,并记录1h内各时间段产生显效的病例数。部分病例做治疗前后的B超检查。结果开始电针后1h,耳穴组的总有效率和显效率皆高于体穴组,两组比较差异有统计学意义(P<0.05)。但开始电针后第2小时,耳穴组的显效率虽仍较体穴组稍高,但两组差异无统计学意义(P>0.05)。开始电针后1h内的各时间段,耳穴组取得显效的例数多于体穴组。开始电针后1hB超检查结果,耳穴组的有效率和显效率皆显著高于体穴组。结论电针耳穴能产生迅速、及时的缓解疼痛效果,电针体穴的缓解疼痛结果较慢,但二者最终结果是相似的。电针耳穴可能主要通过神经系统发生镇痛作用,电针体穴则主要通过某些体液物质。Objective To observe and compare the production time of curative effect between electroacupuncture at auricular points and electroacupunctureat body points in treatment of cholecystalgia, so as to study the different mechanism of auriculoacupuncture and body acupuncture. Methods Twenty six patients in auricular point group were electroacupunctured at right auricurar point ＂Yidan＂ area and ＂Ermigen＂. 24 patients in body point group were electroacupunctured at right body point ＂Danshu＂ and gallbladder tenderness area with same electroacupuncture parameter. The curative effects of one hour and two hours after electroacupuncture began were observed. The cases numbers of marker curative effect in each period within one hour after electroacupuncture were recorded. Part of the patients was tested with B-ultrasonography before and after electroacupuncture. Results One hour after electroacupuncture, the rate of curative effects and rate of marked curative effects of auricular point group was higher than those of body point group, the difference was statistically significant （P 0.05）. Two hours after electroacupuncture began, rate of marked curative effects of auricular point group was just slightly higher than that of body point group, but the difference was not statistically significant （P 0.05）. The number of cases who had marked effects of auricular point group was more than that of body acupoint group in each period within first hour of electroacupuncture began. The test of B-ultrasonography gained same result. Conclusion Electroacupuncture at auricular points can relieve pain speedy and immediately. Electroacupuncture at body points has slower speed in remission of pain. But the final results of the two groups are similar. It is probable that electroacupuncture at auriculo points relieves pain mainly through nervous system, and electroacupuncture at body points mainly works through certain body fluid.
