摘  要:目的观察胫骨癌痛大鼠血脊髓屏障的通透性的变化并初步探讨其机制。方法雌性Wistar大鼠69只,体重160~180 g,随机分为三组,正常组13只,骨癌痛组28只及假手术组28只。正常组不实施手术,骨癌痛组在大鼠右侧胫骨内注射Walker256乳腺癌细胞,假手术组在大鼠右侧胫骨内注射PBS溶液。每组取8只大鼠应用von Frey细丝测定机械性痛阈值的变化,其余大鼠在术后第4、8、12、16天取脊髓腰膨大进行免疫组化实验,检测脊髓腰膨大背角白蛋白免疫阳性细胞数量及星形胶质细胞数量、形态变化。结果正常组大鼠及假手术组大鼠脊髓背角内不含有白蛋白免疫阳性细胞,骨癌痛组造模后第4天脊髓内出现白蛋白免疫阳性细胞(P〈0.01),第16天达到观察中的最高值。与正常组相比,骨癌痛组脊髓背角星形胶质细胞在造模后的第4、8、12、16天明显增多(P〈0.01),胞体肥大,突起增多变粗,相互重叠。与正常大鼠相比,骨癌痛大鼠在注射肿瘤细胞后第2天造模侧出现机械性痛阈值降低(P〈0.05),术后第16天达到观察中的最低点(P〈0.01)。对侧的机械性痛阈也有明显的降低。结论在胫骨癌痛大鼠模型中随着疼痛的进展大鼠出现血脊髓屏障的通透性的增加及星形胶质细胞的激活。Objective To investigate the change of blood-spinal cord barrier permeability in rats with bone cancer and discuss the possible mechanism.Methods 69 female Wistar rats weighing 160 to 180 g were randomly divided into three groups: normal group(n = 13),bone cancer pain group(n = 28) and sham group(n = 28).No operation was performed in the normal group.In bone cancer pain group,rats were induced by right intra-tibial inoculation of Walker 256 mammary gland carcinoma cells.In the sham group,PBS was injected in right tibia.8 rats of each group were chosen to be measured mechanical pain threshold using von Frey hair.The lumbar spinal cords of the rest were removed at 4th、8th、12th、16th day after operation.The number of albumin positive cells and astrocytes,morphological changes of astrocytes in the dorsal horn of lumbar spinal cords were examined by immunohistochemistry.Results Albumin positive cell was not found in the normal and sham groups,which was existed in spinal cord of bone cancer pain group at 4th day after operation(P 0.01),reaching peak at 16th day during observation.Compared to normal group,astrocytes significantly increased in the dorsal horn of spinal cords at 4th,8th,12th,16th day post inoculation of Walker 256(P 0.01)with hypertrophy of cell bodies,apophysis increased and thickening,overlapped.Compared to normal group,tumor-bearing rats displayed a profound decrease in ipsilateral mechanical pain threshold from 2nd day post inoculation(P 0.05),reaching a lowest level at 16th day(P 0.01).The contralateral mechanical pain thresholds significantly decreased too.Conclusion In the present bone cancer pain model,there is a significantly increase in blood spinal cord barrier permeability and astrocyte activation with the mechanical allodynia induced by inoculation of Walker 256.
