摘 要:目的探讨冠脉造影正常女性内皮功能的评价及相关关系。方法选取2006年3月~2008年10月在北京煤炭总医院进行冠状动脉造影正常的女性患者130例,其中慢血流者67例,正常血流者63例。测定其血浆ET-1、NO及内皮舒张功能(FMD),评价血管内皮功能,并分析其相关性。结果静息时,慢血流组和正常组间内皮素-1(ET-1)和NO水平的差异均有统计学意义(P值分别为0.026、0.010)。运动后,两组间ET-1和NO水平的差异均有统计学意义(P值分别为0.031、0.017)。在正常组,运动后ET-1和NO水平均发生明显变化,与运动前比较,ET-1升高而NO降低,差异有统计学意义(P值分别为0.008、0.006)。在慢血流组,运动后ET-1和NO水平变化差异均有统计学意义(P值分别为0.024、0.005)。慢血流组FMD明显低于正常组(P=0.005)。CTFC与FMD呈负相关(r=-0.701,P=0.004),与NO呈负相关(r=-0.850,P=0.009),而与ET-1水平呈正相关(r=0.381,P=0.008)。FMD与血清ET-1水平呈负相关(r=-0.324,P=0.002),与血清NO水平呈正相关(r=0.523,P=0.001)。结论冠脉造影正常的女性患者,冠脉血流和血管内皮功能具有相关性,血管舒张功能下降可能是导致女性冠状动脉硬化形成的主要机制。Objective To evaluate the relationship between coronary blood flow and endothelial function of women with normal coronary angiography.Methods 130 inpatients women with normal coronary angiography in Beijing Coal general hospital from March 2006 to October 2008 were selected,and then they were divided into two groups according to the frames of CTFC,67 inpatients in slow flow group and 63 inpatients in normal group.All of them were performed FMD(Flow-mediated Dilation),examined blood serum NO and the ET-1,and the relationship were analyzed.Results ET-1 and NO was contrast obviously in slow flow group compared with normal group in resting(P = 0.026,P = 0.010 respectively).ET-1 and NO was contrast obviously in slow flow group compared with normal group in post-exercise(P = 0.031,P = 0.017 respectively).In normal group,ET-1 and NO changed obviously after exercise(P = 0.008,P = 0.006 respectively);in slow flow group,ET-1 and NO changed obviously too after exercise(P = 0.024,P = 0.005 respectively).FMD was lowther than that of normal group in slow flow group(P = 0.005).CTFC had a significant negative correlation with the FMD and NO(r =-0.701,P = 0.004;r =-0.850,P = 0.009),and had a significant positive correlation with the ET-1(r = 0.381,P = 0.008).FMD had a significant negative correlation with the change in ET-1(r =-0.324,P = 0.002),and also had a significant positive correlation with the change in NO(r = 0.523,P = 0.001).Conclusion Among female patients with normal coronary angiography,the coronary blood flow has a significant correlation with endothelial function,endothelial dysfunction may be one of the major mechanisms of women coronary arteriosclerosis.
