臂丛麻醉下针刀联合手法松解治疗肩关节周围炎的疗效评价

摘　　要：目的:探讨臂丛麻醉下针刀联合手法松解治疗肩关节周围炎的临床疗效。方法:将106例肩周炎患者随机分为观察组和对照组,每组各53例。观察组均采用臂丛麻醉下针刀联合手法松解,对照组采用传统的针灸和推拿治疗。比较分析两组的临床疗效、疼痛程度、肩关节活动度及日常生活能力变化情况。结果:观察组的临床疗效显著优于对照组(P<0.05);两组治疗后VAS评分均较治疗前显著降低(P<0.05),而治疗后观察组VAS评分显著低于对照组(P<0.05);与治疗前比较,两组治疗后Mallet评分显著降低、ADL显著升高(P<0.05),而治疗后观察组Mallet评分、ADL的改变程度较对照组明显(P<0.05)。结论:臂丛麻醉下针刀联合手法松解治疗肩周炎,可有效缓解肩关节疼痛程度,改善肩关节活动度及日常生活能力。Objective： To explore the clinical effect of acupotomy combined with manipulation under brachial plexus anesthesia for scapulohumeral periarthritis.Methods： 106 patients with scapulohumeral periarthritis were randomly divided into two groups.The observation group（n = 53） was given with acupotomy combined with manipulation under brachial plexus anesthesia for scapulohumeral periarthritis,and the control group was given with traditional acupuncture combined with massage manipulation. The clinical effect,pain degree,improvement of shoulder joint motion and activity of daily living（ADL） of the patients were observed.Results： The clinical effect in observation group was significantly better than that in control group（P0.05）. The VAS scores after treatment in both groups were significantly lower than that before treatment（P0.05）,and there was significant differences between the observation group and the control group after treatment（P0.05）.Compared with before treatment, the Mallet Scores of after treatment in observation group significantly decreased,ADL increased（P0.05）,and there was significant differences between the observation group and the control group after treatment（P0.05）.Conclusion： Acupotomy combined with manipulation under brachial plexus anesthesia on treating scapulohumeral periarthritis can effectively relieve pain of shoulder joint,and can improve the shoulder mobility and ADL.
