摘 要:目的探讨肢体缺血后适应对兔急性肠道缺血再灌注损伤的保护作用及其机制。方法健康雄性新西兰大白兔36只,随机分为3组(每组12只):空白对照组(Con)、肠道缺血再灌注组(IRI)、肢体缺血后适应组(LIP)。再灌注结束后测定血清中丙二醛(MDA)含量,测定肠道组织中髓过氧化物酶(MPO)活性并在光镜下观察肠道组织形态学变化。结果IRI组和LIP组的MDA含量[(8.93±1.25)、(5.32±1.54)μmol/L]、MPO活性[(3.68±0.45)、(1.94±0.51)U/100g]及肠黏膜损伤评分(4.15±0.81、2.96±0.64)均明显高于Con组[(3.96±1.36)μmol/L、(0.56±0.36)U/100g.0.82±0.70),差异均有统计学意义(P〈0.01)。LIP组的MDA含量[(5.32±1.54)μmol/L]、MPO活性[(1.94±0.51)U/100g]及肠黏膜损伤评分(2.96±0.64)明显轻于IRI组[(8.93±1.25)μmol/L、(3.68±0.45)U/100g、.15±0.81],差异有统计学意义(P〈0.01)。结论肠道缺血再灌注前肢体短暂缺血再灌注对肠道急性缺血再灌注损伤有显著保护作用。其保护作用可能与减轻活性氧的损伤及加强抗氧化能力有关。Objective To investigate the protective effect of remote postconditioning induced by a single 10-min episode femoral artery occlusion referfusion applied immediately before the oneset of superior mesenteric artery perfusion on acute intestinal ischemia-reperfusion injury (IRI). Methods Thirty-six healthy New Zealand white rabbits were randomly divided into three groups (n = 12 in each group) : control, IRI, and limb ischemic postconditioning (LIP). Tissue myeloperoxidase (MPO) activity and malondialdehyde (MDA) levels were determined and the morphological changes in intestinal tissues were observed under the microscopy. Results MDA levels, MPO activity and the score of intestinal mucosal injury were signifi- cantly increased in IRI and LIP groups as compared with control group (P 〈 0.01 ). MDA levels and MPO activity were significantly reduced in RLP group [ (5.32 ±1.54) μmol/L, and ( 1.94 ±0. 51 ) U/100 g] as compared with IRI group [ (8.93±1.25 ) μmol/L, and (3.68 ± 0.45 ) U/100 g ] ( P 〈 0. 01 ). The score of intestinal mucosal injury in LIP group was lower than in IRI group (2. 96±0. 64 vs. 4. 15 ±0. 81 ). Conclusion Limb postconditioning protects the mice from acute IRI probable by decreasing the iniury caused by oxygen free radicals and increasing the capability of antioxidation.
