摘　　要：目的:探讨我院3种清洁手术患者预防用抗菌药物的干预效果。方法:选择2009-2011年在我院行疝气、乳腺、甲状腺3种清洁手术的患者共726例,2009年对其进行干预前的基线调查;2010年定期对负责3种清洁手术的医务人员进行抗菌药物合理应用知识培训,并同时开展无高危因素3种清洁手术患者预防用药随机对照试验;2011年采用技术干预和行政干预相结合进行综合干预。比较3年中3种清洁手术患者的不合理预防用药情况和抗菌药物平均使用强度。结果:经对相关医师进行教育培训和综合干预后,我院不合理预防用药发生率、抗菌药物应用率、预防用药时间、使用抗菌药物金额均逐年降低(P<0.01),但采用综合干预比单纯的教育培训和未干预的效果更为明显(P<0.01)。结论:通过积极的综合干预,可明显减少3种清洁手术不合理预防用药发生率,降低抗菌药物使用强度,从而减缓耐药率上升,而且效果优于单纯的教育培训。OBJECTIVE： To explore the effect of antibiotics for prophylactic use on 3 kinds of clean surgery in our hospital. METHODS： 726 patients underwent hernia, mammary gland and thyroid gland clean surgery in our hospital during 2009- 2011 were enrolled and received baseline investigation in 2009; training for reasonable use of antibiotics was carried out among medical personnel who were responsible for 3 kind of clean surgery in 2010 regularly, and prophylactic medication of 3 kinds of clean sur- gery without high-risk factor were studied retrospectively simultaneously; comprehensive intervention was conducted in 2011 through technical intervention combined with administrative intervention; irrational drug.use and AUD of antibiotics were compared among 3 kinds of clean surgery during 2009--2011. RESULTS： After education and training for physicians and comprehensive in- tervention, unreasonable prophylactic drug use, utilization ratio of antibiotics, prophylactic medication time, amount of antibiotics reduced year by year （P〈0.01）, but the effect of comprehensive intervention was better than education and training alone and be- fore intervention （P〈0.01）. CONCLUSION： The incidence of irrational prophylactic drug use in 3 kind of clean surgery is re- duced significantly through positive comprehensive intervention, and AUD of antibiotics is also decreased so as to slow down the in- crease of drug resistance rate. Moreover, the effect of comprehensive intervention is better than that of education and training alone.
