心电编辑技术在室性早搏患者冠状动脉DSCT检查中的应用

摘　　要：目的探讨心电编辑技术在室性早搏患者双源CT(DSCT)冠状动脉成像中的应用。方法搜集2011年9月至2012年1月在本院行DSCT冠状动脉检查的室性早搏患者37例(包括偶发室性早搏15例,频发室性早搏11例,二联律7例,三联律3例,室性早搏伴短阵性室性心动过速1例),联合应用心电编辑技术,评价心电编辑前后图像质量,并进行统计学分析。结果 37例患者中35例编辑成功,右冠状动脉、左冠状动脉前降支及回旋支心电编辑前图像质量分别为2.01±0.82、2.52±0.99、1.65±0.96,编辑后图像质量分别为3.42±0.54、3.26±0.64、3.12±0.41,各支编辑前后图像质量比较均有统计学意义(t=8.95、9.18、10.59,P<0.05);35例心电编辑成功,其中30例(81.08%)使用的是"删后",4例使用的是"删前",1例使用的是"加中间,删两边",同时联合应用绝对值编辑,2例编辑失败。结论在室性早搏患者中,利用"删后"、"删前"及"加中间,删两边"心电编辑方法可以明显减轻或消除心率所带来的冠状动脉CTA伪影,显著提高图像质量;而"删后"是室性早搏患者心电编辑的主要方法。Objective To evaluate the clinical value of electrocardiogram（ECG）-editing technique in dual source CT coronary artery angiography in patients with ventricular premature beats.Methods 37 patients with ventricular premature beats were included into this study.A retrospective gating technique and ECG editing technique were used,two experienced radiologists evaluated in consensus all pre editing and post editing reconstructed image and scores was recorded according to RCA,LAD,LCx.Image quality was assessed by using a four point grading scale from excellent（4 scores） to non assessable（1 score）.Results After editing,35 patients were succeed,the overall mean image quality before and after ECG editing was 2.01±0.82,2.52±0.99,1.65±0.96 and 3.42±0.54,3.26±0.64,3.12±0.41 respectively.The difference between preediting and postediting was significant（t=8.95、9.18、10.59,P0.05）.Long R-R interval was deleted in 30 patients;short R-R interval was deleted in 4 patients;In one case,first insert one point between long and short R-R interval,and then deleted both sides.Conclusion ECG editing technique can improve image quality of coronary artery,because it can reduce and compensate for part of artifacts by ventricular premature beats during CT coronary angiography.Deleting long R-R interval is main ECG editing method in VPBs patient.
