摘 要:目的通过Meta分析评估醛固酮合成酶CYP11B2(-344C/T)基凶多态性与左心室肥厚之间的相关性。方法查阅1992年1月至2011年12月发表的有关醛固酮合成酶CYP11B2(-344C/T)基因多态性与左心事肥厚关系的相关研究文献,数据库包括Embase、Pubmed、CNKI(中剧期刊全文数据库)、CBM(中国生物医学文献数据库)、VIP(维普中文期刊数据库)及万办数据库。以左心室肥厚组和正常对照组基因分布的Oil值为统计量,应用RevMan5.1软件对各研究结果进行异质性检验和相关数据的合并,最终入选6篇病例对照试验文献,为避免文献间的异质性,将纳入文献分为高血压与种族两个亚组进行分析。结果共入选左心室肥厚患者541例,对照组553例。左心室肥厚组与对照组CC/(CT+TT),在i苗血压亚组中OR值1.15(95%CI:0.74—1.80),显著性检验z值为0.63,P=0.53;在种族亚组中OR值1.15(95%CI:0.74~1.80),显著性检验z值为0.63,P=0.53。左心室肥厚组与对照组C等位基因/T等位基因频率OR值1.15(95%CI:0.76~1.74)/0.87(95%CI:O.58—1.31),显著性检验z值为0.67,P=0.50。结论无论是高血压人群或是不同种族人群,醛固酮合成酶CYPI182(-344C/T)基因多态性与左心室肥厚无关。Objective To investigate the association between CYP11 B2 gene polymo- rphism and left ventricle hypertro- phy with meta analysis. Methods Literatures about the association of CYPI 1 B2 gene polymo^-phism and left ventricle hyper- trophy from January 1992 to December 2011 were searched. The electronic databases retrieved from Pubmed,Embase, China national knowledge interact, Chinese biological medicine disk, VIP fulltext database and Wanfang fulltext database. Odds ratio of CYPI 1 B2 genotype distributions in left ventricle hypertrophy patients comparing with healthy control were analyzed. Rev- ManS. 1 software was applied for investigating hereogeneity among individual studies and summarizing effects with proper statis- tical methods. Six case control studies were enrolled. Results A total of 541 cases and 553 controls were enrolled for the stud- y. The pooled OR of CC vs TT + TC genotype was 1.15 (95 % CI : 0.74 - 1.80) ( Z = 0.63, P = 0.53 ) in the subgroup of hy- pertension , and the pooled OR ofCC VSTrF+TC genotypewas 1.15(95%CI: 0.74~1.80)(Z= 0.63, P= 0.53) in the subgroup of race . The pooled OR of C vs T allele was 1.15 ( 95 % CI : 0.76 - 1.74 ) vs 0.87 ( 95% CI : 0.58 - 1.31 ) ( Z = 0. 67, P = 0.50) . Conclusion Whether the hypertension or the race, the genotype of CYPI 1 B2 polymorphism has no asso- ciation with an increased risk of left ventricle hypertrophy.
