摘　　要：目的调查非霍奇金淋巴瘤(NHL)患者乙型肝炎病毒(HBV)感染状况及分析HBV DNA载量。方法分别应用免疫学方法和实时荧光定量聚合酶链反应(PCR)分析130例NHL患者HBV感染状况及HBV DNA载量。130例NHL患者为NHL组,312例其他恶性肿瘤患者为对照组,阳性率比较采用χ2检验。结果 130例NHL患者中HBV感染25例(19.2%),312例其他肿瘤组中HBV感染27例(8.7%),NHL组高于对照组(P<0.05)。按年龄组统计,15～30岁组NHL患者HBV感染阳性率为11.1%,>70～81岁组阳性率为9.1%,>30～40岁组、>40～50岁组、>50～60岁组、>60～70岁组HBV感染的阳性率均大于20%。25例HBV感染者中,HBeAg阳性者4例,HBV DNA载量分析有15例为阳性,其中低载量组5例,中载量组6例,高载量组4例。130例NHL患者中B淋巴细胞型89例,HBV感染阳性率为23.6%,T淋巴细胞型32例,HBV感染阳性率为9.4%。结论 NHL和HBV慢性感染关系密切,HBV可能在NHL的病理发展过程中扮演重要作用,NHL患者HBV感染的特点应该引起临床医师注意。Objective To investigate the prevalence of hepatitis B virus（HBV） infection and HBV DNA loads in non-Hodgkin lymphoma（NHL） patients. Methods HBV infectious status were investigated by using an enzyme-linked immunosorbent assay, and HBV DNA loads were detected by real time PCR-Fluorescence quantification in 130 NHL hospitalized patients. The positive rates were compared by chi square test. Results The prevalence of positivity for HBV infection was 19.2% among 130 NHL patients and 8.7% among 312 other cancer controls. The data analysis showed there was a high prevalence of HBV infection in NHL cases than in other cancers. The age-specific positive rates of HBV infection in different age group NHL patients were 11.1 % （15-- 30 years） and 9.1 % （〉70--81 years）, respectively. However, the positive rates were all above 20% in other age group NHL patients （〉 30 -- 40 years, 〉40 -- 50 years, 〉 50 -- 60 years, 〉 60 -- 70 years）. Four patients were HBeAg positive among 25 N H L pa- tients infected with HBV. The HBV DNA loads of 15 cases were positive by real time PCR-Fluorescence quantification. The 15 cases were divided into three groups according to HBV DNA loads：a low-load group（5 cases）, a median-load group（6 cases） and a high-load group（4 cases）. The positive rates of HBV infection were 23.6 % （B-cell type） and 9.4% （T-cell type） in 130 NHL patients,respectively. Conclusion A positive association was between chronic HBV infection and NHL,and the current data suggested that HBV may play an etiological role in the development of NHL. The clinician should pay attention to the prevalence status of HBV infection in NHL patients.
