摘　　要：[目的]总结乳管内窥镜联合高频超声定位法在乳头状瘤手术病人的围术期护理措施。[方法]将我院收治乳腺导管内乳头状瘤100例病人随机分为对照组和研究组各50例,对照组采用传统乳管扩张器定位法或亚甲蓝定位法行乳头状瘤切除术,研究组采用乳管内窥镜联合高频超声定位法行乳头状瘤切除术,围术期进行心理护理、术前准备、术中护理、术后严密观察病情变化,并发症的预防和护理,健康教育指导等。术后随访6个月～18个月,比较两组病人可正常哺乳者乳汁分泌正常率。[结果]100例病人均成功完成手术,术后无并发症及无乳腺外形改变,均无复发,病人均对手术结果满意。研究组可正常哺乳者乳汁分泌正常率高于对照组,两组比较差异具有统计学意义(P<0.01)。[结论]乳管内窥镜联合高频超声定位法行乳头状瘤手术具有手术损伤小、定位准确、能最大限度满足哺乳期妇女正常产奶的愿望,并能保持乳房对称与完好曲线。Objective： To summarize the perioperative nursing measures of milk ducts endoscope combined with high - frequency ultrasound positioning method in papillary tumor patients undergoing surgery. Methods. A total of 100 cases of patients with breast intraductal papilloma who were treated in our hospital were randomly divided into control group and study group, 50 cases in each, control group patients adopts traditional milk tube expander lo- calization method or methylene blue localization method for papilloma resec- tion, the study group used milk duct endoscope combined with high- frequency ultrasonic localization method for papilloma resection, then the patients were given the psychological nursing,preoperative preparation,intraoperative care, postoperative close observation of condition changes, the prevention and nursing of complications, health education guidance during perioperative period. Patients were followed up for 6- 18 months,then to compare the normal rate of milk secretion of patients with normal lactation between both groups. Results：the operations on all 100 patients were successfully,no postoperative complications occurred and no changed in breast shape, there was no recurrence, patients were satisfied with the surgical results. The normal rate of milk secretion of study group patients with normal lactation was higher than that of control group, the difference was statistically significant between both group（P〈0.01）. Conclusion： Milk duct endoscope combined with high - frequency ultrasonic localization method for papilloma surgery has the advantages including little surgical injury,accurate location,also best to satisfy the desire of lactating women on normal milk production and keeping the breasts symmetrical with the intact curve.
